The Animal Feeding Operation Program of the Division of Water Resources (Division) has completed a
preliminary review of your request for a Zero Animal Permit and permit application package. Additional
information is required before we may continue our review. Please address and submit the following
item(s) within 30 (thirty) days of receipt of this letter:
1. A statement regarding your future intended plans for the facility. This statement should indicate
whether you intend to repopulate the facility in the future and when you expect that might occur.
If you do not intend to repopulate the facility, you should include a proposed plan for closure of
the waste storage structures.
Please reference the subject application number when providing the requested information. Additional
information/or additional documentation shall be signed, dated, and sent to Division's attention at the
address below. The Information/documentation can also be submitted electronically at
