Center - DWR 2015-1097
The Division of Water Resources (DWR) has performed a review of a 401 Water Quality Certification application dated
October 5, 2015 for the above referenced project. The DWR has determined that your application is incomplete and
cannot be processed. The application is on -hold until the following information is received:
1. The impact table and project description state that 40 -feet of 18 -inch culvert pipe will be installed at the
upstream extent of the project. The submitted plan sheet (2 of 5) states "extend existing 18 -inch corrugated
HDPE pipe additional 20 -feet". The scale on the plan sheet confirms a 20 -foot pipe. Please update the impact
table for the reduced impact length on the submitted PCN application. (page 4 of the application)
2. The project description states "Below this area the grading will be done above the ordinary high water mark, so
no permit will be required." However, 3 of the 4 cross-sections on plan sheet 3 show grading down to the bed
of the channel. Please update the plan sheet or account for these temporary impacts in the PCN impact table.
Pursuant to Title 15A NCAC 02H .0502, the applicant shall furnish all of the above requested information for the proper
consideration of the application. If all of the requested information is not received within 30 calendar days of receipt of
this email, the DWR will be unable to approve the application and it will be returned. The return of this project will
necessitate reapplication to the DWR for approval, including a complete application package and the appropriate fee.
