The Division of Water Resources (the Division) became aware of an error in your recently issued permit
(November 30, 2015). The summer (April 1-October 31) and winter (November 1-March 31) limits for
Ammonia monitoring were reversed, with summer having higher limits and winter having lower limits.
Ammonia monitoring limits should be lower for summer months and relaxed for winter months.
To better clarify the minimum limitation for DO as 5 mg/L, the following was added to the effluent table in
Section A. (L):
Daily average not less than 5.0 mg/L
Accordingly, we hereby forward the modified permit pages to amend your permit for Ammonia summer and
winter limits. In addition, the DO limit was noted as a minimum. Please insert these change pages into your
existing permit and discard the old pages. We regret any inconvenience this causes your organization.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Resources or any other Federal, State,
or Local governmental permits that may be required.

Footnotes:
1. No later than 270 days from the effective date of this permit, begin submitting discharge monitoring reports
electronically using NC DWR's eDMR application system. See Special Condition A. (2.).
2. E = Effluent; I = Influent; U = Upstream; and D = Downstream.
3. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance with the permit.
However, the Permittee shall continue to record and submit all values reported by North Carolina certified test
methods (including field certified), even if these values fall below 50 µg/L.
4. Fecal Coliform shall be calculated using the geometric mean, according to the procedure detailed in Part II. Section
A.
5. For a given wastewater sample, TN = TKN + (NO2-N + NO3-N), where TN is Total Nitrogen and TKN is Total
Kjeldahl Nitrogen, and (NO2-N + NO3-N) is Nitrite/Nitrate Nitrogen.
Conditions:
• There shall be no discharge of floating solids or foam visible in other than trace amounts.

A. (2.) ELECTRONIC REPORTING OF DISCHARGE MONITORING REPORTS

1. Reporting [Supersedes Section D. (2.) and Section E. (5.) (a)1
Beginning no later than 270 days from the effective date of this permit, the permittee shall report
discharge monitoring data electronically using the NC DWR's Electronic Discharge Monitoring
Report (eDMR) internet application.
Monitoring results obtained during the previous month(s) shall be summarized for each month and
submitted electronically using eDMR. The eDMR system allows permitted facilities to enter
monitoring data and submit DMRs electronically using the internet. Until such time that the state's
eDMR application is compliant with EPA's Cross -Media Electronic Reporting Regulation
(CROMERR), permittees will be required to submit all discharge monitoring data to the state
electronically using eDMR and will be required to complete the eDMR submission by printing,
signing, and submitting one signed original and a copy of the computer printed eDMR to the
following address:

If a permittee is unable to use the eDMR system due to a demonstrated hardship or due to the
facility being physically located in an area where less than 10 percent of the households have
broadband access, then a temporary waiver from the NPDES electronic reporting requirements may
be granted and discharge monitoring data may be submitted on paper DMR forms (MR 1, 1. 1, 2, 3)
or alternative forms approved by the Director. Duplicate signed copies shall be submitted to the
mailing address above.
Requests for temporary waivers from the NPDES electronic reporting requirements must be
submitted in writing to the Division for written approval at least sixty (60) days prior to the date the
facility would be required under this permit to begin using eDMR. Temporary waivers shall be
valid for twelve (12) months and shall thereupon expire. At such time, DMRs shall be submitted

Removing the footnote for DO and placing the limit in the table was not in error. Its true that the column states
"Daily Maximum". Some permit writers treat this column as more of a daily limits column. Since DO limits are
based on 213 water quality standards where "not less than 5 mg/L" is spelled out, it is a given that what we mean
is "less than" in regard to a limit for DO. However, what I may do is block out the row beside DO much like pH
adding a statement to better clarify the requirement: Daily average not less than 5.0 mg/L. I have seen this
given in a few examples instead of a footnote that does a better job of making the DO limitation clear for the
permittee.

I am writing concerning the above referenced permit which was effective January 1, 2016 (see attached).
A correction for ammonia was also received in a letter dated December 17, 2015.
We have identified another permit error. This concerned the dissolved oxygen. The 5.0 mg/l is shown in the
"maximum" column. The previous permit had nothing in either the monthly average or the maximum but had a
footnote that read "The daily average dissolved oxygen effluent concentrations shall not be less than 5.0 mg/L"
Could we get your assistance to send out a permit revision for this applicable page?
I sincerely regret that we did not catch this earlier.
