the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes no major changes from the draft permit sent to you on May 25,

If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local

Addressing impaired waters is a high priority with the Division, and instream data will
continue to be evaluated. If there is noncompliance with this permit's effluent limits and
stream impairment can be attributed to your facility. then mitigative measures may be
required'.
Process Narrative
Influent flow enters the Roanoke Rapids Wastewater Treatment Plant (WWTP) via two outfalls: the
Chockoyotte Creek Outfall and the River Outfall. Both outfalls combine in a diversion box prior to
mechanical screening and grit removal. Influent flow may be routed to influent equalization, when
necessary. The influent wastewater is lifted via an influent pump station to primary clarifiers. Primary
clarified effluent gravity flows to trickling filters. Trickling filter effluent is pumped to aeration tanks for
further secondary treatment. The mixed liquor then flows to final clarifiers. Clarified effluent is then
chlorinated and dechlorinated. Effluent flow is measured prior to discharge to the Roanoke River.
An effluent flood pump station is used to pump to the Roanoke when flow by gravity may not be
achieved.
The solids handling process description is provided in Attachment C.

INSTREAM MONITORING POLICY
Historical Approach.
It has been the NPDES policy to generally include instream monitoring (i.e., upstream, downstream) for
DO, Temperature, fecal coliform, and conductivity whenever modeling results indicated the potential
for a discharge to reduce the minimum downstream dissolved oxygen levels to within 1.0 mg/1 of the state
water quality standard (6.0 mg/1 for Trout or 5.0 mg/I for non -Trout waters).
Revised 1999 Policy for 100% Domestic Wastewater Dischargers.
Authority is delegated to the NPDES Supervisor to add or delete any or all instream monitoring
requirements. Instream monitoring for fecal coliform and conductivity may not be necessary for 100%
domestic wastewater discharges. Conductivity is a parameter better suited for measuring the impact of
inorganic wastes, and in many cases, instream fecal coliform data can be difficult or impossible to
interpret. These instream monitoring parameters are also not required by EPA. Beginning in 1999,
instream monitoring for fecal coliform and conductivity will not be included in NPDES permits for
100% domestic dischargers, unless the receiving stream is listed on the North Carolina 1998 303(d) list as
an impaired waterbody for fecal coliform bacteria. Not for 100% domestic only for Muni w complex
waste stream or Industrial users.
Defining Instream Monitoring Locations.
When reviewing results of a model, examine the dissolved oxygen (DO) levels in the stream profile. If
the minimum DO concentration (DO sag) occurs elsewhere downstream than at the discharge point, and
the predicted minimum DO concentration is within 1.0 mg/I of the appropriate stream standard, instream
monitoring is normally recommended. When recommending instream monitoring for a facility,
downstream monitoring should be specified as close to the predicted DO sag as possible.
Example L A discharge to a Trout stream is modeled and the following stream profile results. In
this case, the DO sag occurs at the discharge point, and the stream assimilates the waste readily.
Although about 100 feet downstream the DO is still within 1.0 mg/I of the standard, no DO sag is
apparent downstream, so no instream monitoring would be recommended.

They sold it. The new owner is Chuck Mervin 803-487-4242. The contact person is Eva at 704-604-4790. Apparently
they are not paying WQ lab and operations either and Jadd said they may quit servicing the plant. I told Jadd if they did
they needed to send in a letter stating this, but have not seen anything so far. This is a tiny plant and there is currently
no flow at all as the new owner was remodeling the entire building and there are no residents but what was in the plant
is just sitting there. It needs to be pumped but I haven't been able to get up with anyone to date.
