Dear Mr. Gurowski:
The Division received your application for a wastewater discharge permit on December 15, 2004. Division
personnel have reviewed and approved your application- Accordingly, we are forwarding the attached NPDES
discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 2004, and as subsequently amended.
A Total Residual Chlorine (TRC) limit has been added to this permit. Because you may need time to make
modifications to your plant in order to comply with this new condition, the TRC limit will not take effect until
February 1, 2007.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of
the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, North Carolina 27699-6714. Unless such demand is made, this decision shall be final and
binding.
Please note that this permit is not transferable except after notice to the Division, The Division may require
modification or revocation and reissuance of the permit.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.
If you have any questions concerning this permit, please contact Vanessa Manuel at telephone number

Re: NPDES Permit NCO036935
Gentlemen:
We hereby request renewal of the captioned permit. Since the filing of the previous
permit renewal application dated February 29, 2000, the dual train activated sludge
system with a permitted flow of 0.0696 MGD has been replaced with a Package Style
Wastewater Treatment System with a design flow equal to 20,000 GPD @ 240 PPM
BOD5 manufactured by Tipton Environmental International, Inc. of Batavia, Ohio.
Authorization to Construct Permit Number 036935A01 was approved as filed by letter
dated September 24, 2003 signed by Alan W. Klimek, P.E.
The facility has an aerated sludge digester in which the waste sludge is stored and
disposed of from time to time. The sludge is removed and disposed of by Mulls Concrete
& Septic Tank Service, 2416 Mount Home Church Road, Morganton, NC 28655 in
accordance with Burke County regulation.

Hi Jim, I am reviewing the permit renewal for Pine Mountain Lakes WWTP (NC0036935) in
Burke County, and they indicate they have replaced their 69,600 gpd dual train system
with a 20,000 gpd package plant. I would like verification from the region that we
should reduce their design flow in the renewed permit. A reduced flow may also reduce
their monitoring requirement. Thanks. -Vanessa
