Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or
as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on April 14,
2004.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such
demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required. If you have any questions concerning this

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of this permit
issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive authority to
operate and discharge from this facility arises under the permit conditions, requirements, terms, and provisions included
herein.
Lucent Technologies, Inc. is hereby authorized to:
1. Continue to operate an existing 0.302 MGD groundwater remediation treatment system
consisting of the following components:
• Extraction wells
• Oil/water separator
• Granular media filtration
• Feed -water storage and equalization
• Parallel filtration and air -stripping treatment trains receiving segregated flow
(System A receives flow from diesel fuel extraction wells and perimeter VOC extraction
wells; System B receives flow from interior VOC extraction wells)
• Activated carbon
• Solids removal (filter press)
The facility is located at Salem Business Park, 3370 Lexington Road, Winston Salem,
Forsyth County.
2. Discharge from said treatment works at the location specified on the attached map into
Salem Creek, Class C water in the Yadkin -Pee Dee River Basin, at the location specified
on the attached map.

SUPPLEMENT TO EFFLUENT LIMITATIONS
AND MONITORING REQUIREMENTS
SPECIAL CONDITIONS
A (2). CHRONIC TOXICITY PASS/FAIL PERMIT LIMIT (QUARTERLY)
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to Ceriodaphnia
dubia at an effluent concentration of 90.0 %.
The permit holder shall perform at a minimum, uaq rterh monitoring using test procedures outlined in the "North Carolina
Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or "North Carolina Phase
II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions. The tests will be
performed during the months of March, June, September, and December. Effluent sampling for this testing shall be performed
at the NPDES permitted final effluent discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit, then
multiple -concentration testing shall be performed at a minimum, in each of the two following months as described in "North
Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the'highest concentration
having no detectable impairment of reproduction or survival and the lowest concentration that does have a detectable
impairment of reproduction or survival. The definition of "detectable impairment," collection methods, exposure regimes,
and further statistical methods are specified in the "North Carolina Phase If Chronic Whole Effluent Toxicity Test Procedure"
(Revised -February 1998) or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge Monitoring Form
(MR-1) for the months in which tests were performed, using the parameter code TGP3B for the pass/fail results and THP3B
for the Chronic Value. Additionally, DWQ Form AT-3 (original) is to be sent to the following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Branch no later than 30 days after the
end of the reporting period for which the report is made.
Test data shall be complete, accurate, include all supporting chemical/physical measurements and all concentration/response
data, and be certified by laboratory supervisor and ORC or approved designate signature. Total residual chlorine of the
effluent toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the permittee
will complete the information located at the top of the aquatic toxicity (AT) test form indicating the facility name, permit
number, pipe number, county, and the month/year of the report with the notation of "No Flow" in the comment area of the
form. The report shall be submitted to the Environmental Sciences Branch at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be required
during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water Quality
indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include alternate monitoring
requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism survival,
minimum control organism reproduction, and appropriate environmental controls, shall constitute an invalid test and will
require immediate follow-up testing to be completed no later than the last day of the month following the month of the initial
monitoring.

1.0 Proposed Changes Incorporated into Permit Renewal
• Remove limit and monitoring for 1,1 Dichloroethene,1,1,1-Trichlorothane, carbon
tetrachloride, chloroform and iron due to lack of reasonable potential for these parameters to
effect water quality.
• Remove limit for trichloroethene due to lack of reasonable potential for this parameter to
effect water quality. Monitoring will remain as this is still considered a parameter of concern.
2.0 Summary
This is a minor permit renewal for an existing 0.302 MGD groundwater treatment
system. The facility is a pump and treat groundwater remediation system and three coil vapor
extraction systems. The pump and treat system utilizes two oxidizing filters, two carbon filters
and three air strippers in the treatment of groundwater pumped from fifteen extraction wells.
Monthly effluent data for the past 36 months (1/01-12/03) have shown no detections (all
<1 pg/1) for four of the seven toxicants of concern. In addition, quarterly chronic toxicity test
results have passed all events (16/16) since 2000.
The facility discharges to an unnamed tributary to Salem Creek. Salem creek is currently
on the 2002 303 (d) list for fecal coliform, turbidity, and sediment. Nonpoint sources were
identified as the major pollutant sources.
The permittee submitted their NPDES permit renewal application (Short Form C-GW) on
01/2/03. The current permit expires on 6/30/04. NPDES recommends renewal of the current
permit with the changes noted above.

3.0 Compliance Summary

DMR data was reviewed for the period of January 2001-December 2003. The facility is
not required to perform instream monitoring. Effluent monitoring data for flow, TSS, and oil and
grease were reviewed and showed no exceedances of the permit limitations. The facility also
monitored for pH, though it was not a permit requirement. Table 1 summaries average monthly
data for the review period.

RPA Analysis
RPAs were performed for seven parameters. 1,1 dichloroethene,1,1,1- trichloroethane,
carbon tetrachloride, chloroform, and iron showed no detections (less than 1.0 )ig/L and less than
0.05 mg/L for iron) over the 2001-2003 review period (14 quarterly measurements). Limits and
monitoring requirements will be removed for these parameters. Trichloroethene has a limit of
155.0 pg/L and the RPA analysis showed a maximum predicted concentration of 17.9 pg/L. The
limit will be removed for this parameter. Tetrachloroethene has a permitted limit of 17.0 pg/L
and an RPA revealed the maximum predicted concentration to be 22.7 ixg/L. The limit and
monitoring requirement will be retained in this renewal.
WET Test Results
The Salem Business Park has passed all 16 WET tests administered from 2000-2003.
Correspondence File Review/Compliance History

On behalf of Lucent Technologies Inc. (Lucent), ENSR Consulting and Engineering (NC) Inc.
(ENSR) is requesting renewal of NPDES Permit NCO080853 for the groundwater treatment system
located at the former Lucent facility, now known as the Salem Business Park, in Winston-Salem,
Forsyth County, North Carolina. No changes or additions have been made to the treatment system
as covered by the current permit. Two additional extraction wells have been added on the subject
property.
The treatment system consists of extraction wells, oil/water separator, granular media filtration, feed -
water storage and equalization, parallel filtration and air stripping treatment trains receiving
segregated flow (System A receives flow from TPH wells and perimeter extraction wells while
System B receives flow from interior VOC extraction wells) activated carbon, and solids removal (via
filter press).
This facility is a groundwater remediation system and does not have a Sludge Management Plan,
however, a small volume of solids are generated and managed as hazardous waste. Approximately
every four months sludge is pumped from the settling tank (ST-157) through the filter press (FP-
160). After drying in the filter press the filter cake is placed in a labeled, covered 55 gallon drum.
The quantity generated per filter press run is generally less than a full dram. Once a full drum has
been accumulated the drum is sealed and moved from the Satellite Accumulation Area to the
Temporary Storage Area. Within 90 days of being moved to the Temporary Storage Area the
drummed filter cake is picked up, transported and disposed of by ONYX Environmental Inc. Annual
generation of filter press solids is approximately five drums or 2,250 lbs.
Attached is a letter from Lucent dated November 21, 2000, delegating signature authority for NPDES
correspondence to the ENSR Project Manager. Also attached are the required application and
documentation necessary for renewal of the permit. If you have any questions regarding this

For petroleum -contaminated sites, analyses for volatile organic compounds (VOCs) should be performed. Analyses for any fuel additives
likely to be present at the site should also be performed. At minimum, analyses should be performed for the following compounds:
benzene' toluene' ethylbenzene' xylene' lead methyl tert-butylether (MTBE) dibromoethane (EDB)
1,2-dichloroethane isopropyl ether napthalene phenol
('an EPA -approved method capable of detection levels to 1 ppb should be used to detect these compounds).
For solvents or other contaminants, an EPA Method 624/625 analysis should be performed. The data submitted with this application
should include the ten largest peaks not identified as targeted compounds and not present in the procedural blank. These peaks should be
identified and approximately quantitated.
If metals or pesticides are suspected to be present, these compounds should be analyzed to the same detection level as presented in the
NC APAM.
