Winston-Salem, NC 27102 095A �EHNR Subject: Permit No. AC0080853 Authorization to Construct AT&T Lexington Road Facility Wastewater Treatment Facility Forsyth County Dear Mr. Cambell: A letter of request for an Authorization to Construct was received December 16, 1993 and was issued by the Division on March 29, 1994. This letter serves as an amendment to this authorization to construct. Authorization is hereby granted for the construction of a 0.302 MGD wastewater treatment facility to treat contaminated groundwater with the following additions to the March 29th approval; an In Situ Bio-remediation system consisting of phosphate and nitrogen addition to recycled effluent, infiltration galleries, a bio-venting system and necessary piping and pumps for proper operation. Approval is also granted for the optional use of skimmer pumps should it turn out no free product occurs at this site. Should any amount of free product be found on site in accordance with the groundwater remediation occurring, the skimmer pumps shall be installed for removal of the free product. PLEASE NOTE: The approval of the In -Situ Bio-remediation system is conditional on receipt of approval of design yy the Solid and Hazardous Waste Division of the Department of Environment, Health and Natural Resources. Should the approval of the In -Situ Bio-remediation system be denied by Solid and Hazardous waste then Permits and Engineering's approval shall be automatically revoked. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NCO080853 issued March 18, 1994, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0080853. The sludge generated from these treatment facilities must be disposed of in accordance with G.S. 143-215.1 and in a manner approved by the North Carolina Division of Environmental Management. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Petmittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. The Winston-Salem Regional Office, telephone number 910-896-7007, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. P.O. Mail the Certification to the Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of Title 15A, Chapter 8A, .0202. Once the facility is classified, the Permittee must submit a letter to the Certification Commission which designates the operator in responsible charge within thirty days after the wastewater treatment facilities are 50% complete. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the ,o•t, life of the facility. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other govemment agencies (local, state, and federal) which have jurisdiction. Authorization is hereby granted for the construction of a .302 MGD wastewater treatment facility to treat contaminated groundwater. This facility includes four systems: System A includes six (6) groundwater wells, a feedwater tank with the addition of caustic and permanganate, oxidizing filter and a 100 gpm packed tower air stripper; System B includes five (5) groundwater wells, a feedwater tank with caustic and permanganate addition, an oxidizing filter, a 125 gpm packed tower air stripper with a duct heater and post heat exchanger, an optional 125 gpm shallow tray air stripper with a duct heater and the identical heat exchanger from the first air stripper; System C includes four (4) groundwater wells, an adsorption media filter, and connects into the feedwater tank of System A; System D includes a settlinglineutralization tank with polymer flocculant addition and connects into the feedwater tank of System B. System A and B connect prior to the last stage which is a 209 gpm granular activated carbon tank with 6500 pounds of GAC. The total system ends with a treated water holding tank with discharge of treated wastewater into an unnamed tributary of Salem Creek. This authorization to construct approval includes all that is listed above and including all piping, valves, electrical fixtures and appurtenances needed for proper operations of the treatment facility. Please provide a description of proposed wastewater treatment facilities: Three remediation processes and one byproduct process: System "A" - VOC groundwater wells; feedwater tank; oxidizing filter; packed tower air stripper; carbon treatment; treated water tank from which recycle to systems "A" & "B" will occur along with discharge to outfall structure. System "B" - VOC groundwater wells with Acetone; feedwater tank; oxidizing filter; packed tower air stripper; heat exchanger; gas fired boiler; shallow tray air stripper; heat exchanger; with discharge into system "A" just before carbon treatment. System "C" - TPH groundwater wells; adsorption media filter; with discharge into system "A" feedwater tank. Tedder's letter November 22, 1993, which provided a "Draft" copy of Permit No. NC0080853 and notification of the comment period which ends on December 31, 1993. Section "A" of the draft permit establishes a 90% Pass/Fail Chronic Toxicity effluent limitation. In agreement with our telephone conversations of December 13, 1993 regarding the removal of the Chronic Toxicity limitation, we request the removal of this limitation. Our request is based on your advice that normal policy, for your office, does not requiring Chronic Toxicity Testing of groundwater treatment effluents which receive carbon treatment prior to discharge. I have been advised by Mr. Lee Spencer with the Winston-Salem Regional Office of DEM that his report to your office, regarding the renewal of this permit, supports the removal of this permit condition. We have reservations about a pass/fail testing requirement which is subject to a wide range of variabilities in normal groundwater parameters and in sample collection and analysis.
