Madison County
A letter of request for an Authorization to Construct was received.
October 25, 2001 by the Division. Final plans and specifications for
the subject project have been reviewed and found to be satisfactory
contingent upon the conditions and terms stated in this document.
{1) The Madison County Board of Education is still constrained by the
terms and conditions of the current NPDES permit number NC0080659.
{2} Discharge of wastewater from the new Brush Creek Elementary School
may commence ONLY after the authorization to construct (ATC)
conditions described in this document have been completed.
This Authorization to Construct (ATC) is hereby granted for the
construction, installation and modification of the following items:
{1} the addition of a new, separate grease trap to serve the new Brush
Creek Elementary School wastewater discharge,
{2} the addition of a new 10,000 gallon recirculation tank,
{3} the conversion of the existing 2,550 square feet intermittent sand
filter to a recirculating sand filter by adding a distribution box
with a recirculation line (1:1 ratio),

{4} the conversion of the existing pump tank to additional
settling, the installation of a 4-inch pvc recirculation
line to the pump tank, 16
{5} the removal of dual pumps and relocating them to the
proposed recirculation tank, the re -connection of these
pumps to the existing line leading to the sand filter,
{6} new electrical connections to pumps and controls per State
and NEC codes,
{7} removal of sludge contaminated sand (6-12") and replace
with State certified sand -filter grade sand, leveled for
uniform distribution,
{8} the reconstruction and leveling of low pressure pipe distri-
bution lines to allow uniform distribution of wastewater
over the entire sand filter surface,
{9} the addition of an electro-mechanical timer (10 minute
intervals) to the external control panel to control the
dosing/resting intervals of wastewater application (rather
then demand cycles),
{10} the addition of a 15-inch pvc effluent filter'(s) to the
existing pump tank, and
{11} replace or extend the chlorinator and dechlorinator chemical
feed tubes to a level that can be reached to restock while
standing on the ground surface (without entering confined
space through the entrance hatches)
The Division of Water Quality's Asheville Regional Office,
telephone number 828-251-6208, shall be notified at least forty-
eight (48) hours in advance of operation of the installed facili-
ties so that an in -place inspection can be made. Such notifica-
tion*to the DWQ Regional Supervisor or DWQ county contact shall
be made during normal office hours from 8:00 am until 5:00 pm on
Monday through Friday, excluding State Holidays.
Upon completion of construction, installation and
modification and prior to operation of this permitted facility, a
certification must be received from a professional engineer

certifying that the permitted facility has been installed in
accordance with this Authorization to Construct and the approved
plans and specifications. Mail the Certification to the NCDENR-
DWQ, NPDES Unit, 1617 Mail Service Center, Raleigh, NC 27699-
1617.
Failure to abide by the requirements contained in this
Authorization to Construct may subject the Permittee to an
enforcement action by the Division of Water Quality in accordance
with North Carolina General Statutes 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not
preclude the Permittee from complying with any and all statutes,
rules, regulations or ordinances which may be imposed by other
government agencies (local, state and federal) which have
jurisdiction.

as a duly registered Professional Engineer in the State of North Carolina, having been
authorized to observe (periodically, weekly, full time) the construction of the project named,
Madison Middle School located at 1 Turning Point Place in Marshall, Madison County, North
Carolina,
for the Permittee hereby state that, to the best of my abilities, due care and diligence was used
in the observation of the construction such that the construction was observed to be built within
substantial compliance of this authorization to ponstruct and other supporting materials.

The Engineer's Certification shall be completed and submitted to the address above with one
copy of the "Construction Record Drawings" of the wastewater treatment plant modification as
well as supporting design calculations for any pump stations permitted as part of this project.
This project shall not be considered complete nor allowed to operate until this Engineer's
Certification and all required supporting documentation have been received by the Division. Any
wastewater flow made tributary to the wastewater system prior to completion of this Engineer's
Certification shall be considered a violation of the permit and shall subject the Permittee to
appropriate enforcement actions.
