DATE ignature of Official Printed Name ° Title NARRATIVE SUMMARY GENERAL INFORMATION: GENERAL PROGRAM INFORMATION: During this reporting period, the POTW modified the Long-Term Monitoring Plan to include revisions made to the NPDES Permit that was reissued with an effective date of May 1, 2023. The State confirmed receipt of the revised LTMP on November 6, 2023. The POTW did not modify any of the other Pretreatment Program Elements. The POTW reviewed the information on the attached "Pretreatment Program Info Database"and noted an update to the contact information. GENERAL PERMIT INFORMATION: During the reporting period, the City issued industrial user permits with an effective date of October 1, 2023 to the City's water treatment plant and landfill as required by the NPDES Permit. The Permits were submitted to the State for review and approval on August 22, 2023. The Permit issued to the Landfill was approved by the State by email dated July 28, 2023. The State requested a revision to the Permit issued to the Water Treatment Plant to include monitoring for both wastestreams from the water treatment plant and increase the monitoring frequency for TSS to weekly. The UP was revised and resubmitted to the State. SIU INFORMATION: 1. Albemarle MSW Landfill, IUP#0017 SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: None. OTHER INFORMATION: During this reporting period, the Landfill exceeded the daily maximum flow limit in December, 2023. A Notice of Violation was sent to the SIU in accordance with the City of Albemarle Enforcement Response Plan. The POTW completed the sampling and monitoring as required by the Permit. Also, an on-site inspection was completed by the POTW. 2. Hwy 52 Water Treatment Plant, IUP#0018 SIGNIFICANT NONCOMPLIANCE (SNC) INFORMATION: No SNC Events. OTHER INFORMATION: During this reporting period, the Water Treatment Plant met all of the permit limitations and conditions. The POTW completed the sampling and monitoring with the exception of BOD, COD, and Ammonia. These pollutants were tested during the permitting process with a sample date of April 20 and 24, 2023. The laboratory report is attached. These pollutants do not have UP limits and does not pose any adverse affects to the POTW. The POTW will "makeup" the missed samples during the first quarter of 2024. Also, an on-site inspection was completed by the POTW. . It requires annual testing for priority pollutants. They tested the wastewater discharge for priority pollutants in April 2023 for the permit application. Can those results be used to meet the permit requirement?
