Klimek, P.E., Director October? 2 4 Subject: Modification of NPDES Permit NCO081256 FrankL. Ward WTP Guilford County The Division has reviewed and approved your request to modify the subject permit. This modification allows for the temporary use of outfall 002, an alternate outfall from the North Lagoon at your facility. This modification is for temporary use of 002 only. Outfall 002 is only to be used when outfall 001 is unavailable due to maintenance or similar operations. At no time may your facility discharge from both outfalls. Limits and monitoring requirements apply to any discharge from either outfall. No other changes to the permit have been made. Please find enclosed the revised pages, which should be inserted into your permit. The old pages may then be discarded. All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency. If any parts, measurement frequencies or sampling requirements contained in this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding. As of this permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and provisions included herein. The City of High Point is hereby authorized to: 1. Continue to operate a water treatment plant with a discharge of filter -backwash wastewater. This facility is located in High Point at the Frank L. Ward WTP on East Kivett Drive in Guilford County. 2. Discharge from the South Lagoon via outfall 001 at the location specified on the attached map into an unnamed tributary to Richland Creek, classified C waters in the Cape Fear River Basin. 3. Discharge from the North Lagoon on an as -needed basis via outfall 002 [ca. 400 feet upstream of outfall 0011 into an unnamed tributary to Richland Creek, classified C waters in the Cape Fear River Basin. EFFLUENT LBUTATIONS AND MONITORING REQUIREMENTS During the period beginning on November 1, 2004 and lasting until expiration, the Permittee is authorized to discharge from outfall 001 or 002. Discharge may occur from only one outfall. Simultaneous discharge from both outfalls is prohibited. After talking to Cory Basinger in the Winston Salem regional office and Susan Wilson in the Raleigh office, it was suggested that I fill out this application and give you a detailed explanation of what we wanted to do. We have 2 alum sludge lagoons in which we put our treatment solids. At the present time our south lagoon is fulL what we would like to do is to permit another discharge point on the north lagoon until we can get the south lagoon cleaned. This point would only be used temporarily or if we needed to do maintenance on the other lagoon. At no time would we use both discharge points simultaneously. The temporary discharge point would be approximately 400 ft. up stream of the original point and would be sampled as permit requirements dictate. A diagram has been enclosed to illiterate its relation to the original discharge point. We would call it 002. If you have any other questions, please contact me at 336-883-3410. Thank you.
