Inspection Summary:
I spoke with Kerry Dennis, the Plant Manager and he showed me around the site. The facility is still working on additions
located on the West side. The facility mixes and packages chemicals for fertilizer sales.
There are poly tanks of waste oil stored outside that are requird to be located inside and/or in secondary containment if it
meets the size criteria (the storage capacity of the containers, not how much is normally stored in them)
The ditch behind the facility has been greatly contaminated by the fertilizer from the facility. More than likely from the area
on the railroad tracks where product is delivered.This ditch will be required to be cleaned out and the material properly
disposed of once confirmation samples have been taken. Samples should be taken along the ditch until all contamination
has been removed.Please provide the report of the cleanup to the office when completed.
It should be confirmed that the pipe plug in the ditch has been permanently attached, so it cannot be removed.
Review the outfall areas listed in the checklist to determine if one or both should replace the proposed sample point.

Comment: The outfall was inspected and it appears the outfall location is for the ditch that runs by the
parking lot out to the road that doesn't envolve any industrial activity, but only vehicle parking.
There is an outfall located on the NE corner of the facility that would be better suited as an
outfall that outlets into a ditch that runs along the back of the property Location-
(36/10/15.56,-77/40/40.16)There is a pipe outlet on the SE corner of the facility that drains an
area under the railroad tracks, where chemicals are dumped on a conveyor system, that is said
to be capped off. That is another possible outfall location (36/10/12.91,-77/40/42.97).
