On October 2, 2014, an Individual 401 Water Quality Certification was issued under DWR Project No. 13 -0708
V2. On July 22, 2015, this Office issued a Notice of Deficiency for unauthorized impacts to 401 wetlands. On
October 1, 2015, this Office received a request from George Wood, Environmental Professionals, Inc., agent
for Ms. Elizabeth E. Letrendre, to modify the 401 Certification request.
Based review of the available information, this Office will has the following comments:
• To date, no information has been provided regarding the restoration and /or permitting of
unauthorized 401 wetlands fill at the subject site.
• This Office will require the submittal of a modification request to the Individual 401 WQC be submitted
for impacts associated with Minor Modification request.
Pursuant to Title 15A NCAC 02H .0506 (a)(7) and (b)(2), this request to modify the existing 401 certification
will be not be approved until the applicant furnishes all of the above requested information for proper
consideration of the modification request.
