In accordance with the application received on December 1, 2010, the supporting data received February 4, 2011, the
request for modification received August 22, 2011, and the request for permit extension received March 15, 2013, we are
forwarding herewith extended permit number WI0800219. The expiration date has been extended from February 281h
2013 to February 28th, 2014. All other portions of the permit remain valid and intact.
This modified permit shall be effective from March 28, 2013 until February 28, 2014, and shall be subject to the
conditions and limitations stated therein, including the requirement to submit a final project evaluation as stated in PART
VII — MONITORING AND REPORTING REQUIREMENTS. Please read the entire permit to ensure that you are aware
of all compliance requirements of the permit.
You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to
continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew
the permit 120 days prior to its expiration date. Please contact me at (919) 807-6352 and at david.goodrich@ncdenr.gov
