Dear Mr. Townson: Dee Freeman Secretary In accordance with the application received on December 1, 2010, the supporting data received February 4, 2011, and the request for modification received August 22, 2011, we are forwarding herewith the attached two -page modification for permit number WI0800219. These two replacement pages reflect your request to install and utilize 54 permanent injection wells instead of 101 push points for the injection of the remedial substances. All other portions of the permit remain valid and intact. Please replace pages 1 and 2 of the existing permit (dated March 28, 2011) with the attached pages 1 and 2 (dated September 8, 2011). It is the Permittee's responsibility to ensure that a complete permit is created by replacing the pages of Permit No. WI0800179, issued on March 28, 2011, with these modified pages. This modified permit shall be effective from the date of issuance until February 28, 2013, and shall be subject to the conditions and limitations stated therein, including the requirement to submit a final project evaluation as stated in PART VII — MONITORING AND REPORTING REQUIREMENTS. Please read the entire permit to ensure that you are aware of all compliance requirements of the permit. You will need to notify this office by telephone 48 hours prior to initiation of operation of the facility. In order to continue uninterrupted legal use of the injection facility for the stated purpose, you must submit an application to renew the permit 120 days prior to its expiration date.
