Dear Mr.Wagner:
On February 21, 2024, Donald Smith, and Danny Smith from the Raleigh Regional Office of the Division of
Water Resources conducted a Compliance Evaluation Inspection(CEI)of the Terrible Creek Wastewater
Treatment Plant located at 5812 Hilltop Road in Fuquay-Varina. The purpose of this visit was to review the
facility's compliance with the subject NPDES permit. During the inspection,the presence and cooperation of
Adam Stepheson,Wastewater Treatment Plant Supervisor(ORC),Troy King,Backup ORC, and Heather
Adams,Assistant Public Utilities Director were greatly appreciated and very helpful to this review process.
The Terrible Creek WWTP is located along Hilltop Road, in Fuquay-Varina,Wake County. The facility is a
Grade IV Biological Water Pollution Control System with an as-built and permitted flow of 3 MGD with
approval to expand to 6 MGD.
Through compliance with NPDES permit(NC0066516),the Town of Fuquay-Varina is authorized to discharge
treated wastewater through outfall 001 to receiving waters designated as an unnamed Terrible Creek a
waterbody classified as C Nutrient Sensitive Waters(NSW)within the Neuse River Basin.
This NPDES compliance evaluation inspection and requested follow-up response consist of the following:
• Review of the NPDES permit;
• Review of the owner/facility information;
• Review of November 2021-October 2023 monitoring data and compliance history;
• On-site inspection of the wastewater treatment units;
• On-site inspection of the discharge outfall; and
• Request for written response from the Town of Fuquay-Varina.
Findings during the pre-inspection file review are as follows:
1. The current NPDES permit(NC0066516)was issued effective June 9, 2021, and will expire April 30, 2026.
The permittee is not authorized to discharge after the expiration date. To request authorization to discharge
after the expiration date,you will need to submit a permit renewal application at least 180 days prior to the
expiration of the permit.
2. The treatment component, as listed in the subject NPDES permit, includes the following:

Influent Pump Station; Mechanical coarse bar screen;Mechanical step bar screens
manual bar rack, Grit Removal,Flow distribution box,Two(2) 5-stage conventional BNR basins; Two(2)
clarifiers, RAS pump station,two(2)ultra-screen disc filters,UV disinfection, Effluent flow measurement,
WAS pump station, One (1)aerated sludge holding tank; Two (2) 3-belt filters presses with gravity
thickening ability and stand by power.
Not listed in the permit were the following treatment system components: Three equalization basins. [two
(2)250,000 gallon and one(1) 500,000 gallon equalization basins.] These EQ basins were operational but
not in use at time of the inspection.
3. Since the implementation of EPA's NPDES Electronic Reporting Rule,the City has been submitting its
discharge monitoring reports(DMRs)electronically, instead of on paper. The first DMR the Division
received electronically from the permittee was for June 2014.
Please note:As of October 7,2019, all NPDES discharge monitoring reports submitted in the eDMR
system are accepted with electronic signature,signed hardcopies are no longer required to be mailed to
the Division.
4. The Town has designated Adam Stephenson as the facility's ORC and two(2)operators, Chris Grimes and
Troy King,as the backup ORCs. All designated operators have a Grade 4 Certification, and all have an
active certification status.
5. For the review period November 2021 through October 2023,no DMR limit violations were detected. Also,
no penalty assessments have been issued since December 2018.
6. Special Condition A.(4.)of the subject permit requires the permittee to perform Effluent Pollutant Scans
during calendar years 2023,2024 and 2025, and the samples must be collected in different calendar quarters
to represent seasonal variation. The Town has conducted 2023 analysis and plans/processes are in place to
meet the balance of the Effluent Pollutant Scan permit requirements.
7. Special Condition A.(10.)of the subject permit requires the permittee to develop and implement a Mercury
Minimization Plan(MMP)within 180 days of the permit effective date and have available for inspection on-
site.
As requested during the inspection, Heather Adams provided RRO an electronic copy of the MMP for
review on February 26, 2024.
8. The required Annual Administering and Compliance Monitoring Fee for the current billing period was paid.
There are no overdue fees.
9. Permittees who own or operate facilities that collect or treat municipal or domestic waste are required to
provide an annual report to its users/customers summarizing the performance of the collection or treatment
system. The report shall be provided no later than sixty days after the end of the calendar or fiscal year.
The Town's FY 2022-2023 Annual Sanitary Sewer System Report is available to its customers via the
Town's website. It was also explained that this report is available at the Public Service Center and in

Quarterly Newsletter. The content of the report meets the minimum regulatory requirements and educates
its users on the Town collection and treatment systems.
Findings during the inspection were as follows:
10. The ORC maintains a bound,paginated logbook to record daily plant operations.
11. Meritech Inc. (certification number 165), is certified for and conducts the analyses for all required
parameters, except DO,temperature, and pH. The Town conducts and is field certified for DO,
temperature, and pH(field certification number 5527).
A cursory inspection of the meter calibration logs and standards showed the laboratory to be compliant with
record-keeping requirements. (Note: combined pH meter and DO meter was off-site and in use during the
inspection.)
The thermometers used for the self-monitoring program are NIST certified and are replaced annually.
12. The inspector compared the February 2023 and November 2023 eDMR data against the bench lab data and
analytical results. No discrepancies were noted.
13. During the walk-through of the wastewater plant,the inspector observed the treatment units listed in item 2
above. The inspector noted the following observations of the treatment components:
a) Preliminary Treatment. The Preliminary Treatment Facility(PTF) consisting of the-Influent Pump
Station;mechanical step bar screens,manual bar rack, grit removal, flow distribution box,were in
operation and did not appear to have any issues that would affect proper operation. (Note: One of two
grit removal trains were in operation during inspection. The ORC explained that flows are switched over
every quarter and maintenance is performed on the respective offline grit removal train during this
period.)
The area around these units were clean and well maintained. The ORC informed the inspector that the
screenings and grit are collected in a dumpster and hauled to the landfill for disposal. No issues of
concern were observed with the preliminary treatment.
b) Primary Treatment/Secondary Treatment. Plant staff use Two(2) 5-stage conventional BNR
basins; Two(2)clarifiers.
One of the BNR units(BNR unit#2)was offline due to problems with the mixers. Problems were noted
with the mixers,blades, and mounting assemblies. Mounting assembly repair parts were delivered to
WWTP facility during the inspection and repairs are schedule to begin Monday,February 26t1i. Similar
issues with the mixers are also a concern with BNR unit#1. The facility is continuing to work with
their manufacture and their engineers,to affect repairs to BNR basins that remain under the
manufacture's warrantee.
One of the two secondary clarifiers(clarifier number#1)was out of service for repairs due to issues
with the plate in the center column.
Clarifier number#2 is approximately 20 feet deep and was in operation. During the inspection,the
Backup ORC measured the sludge level in clarifier#2 at approximately 3.0 ft. The depth of this

clarifier approximately 20 ft. The inspector observed some pin floc and floating solids/foam within the
unit.
c) Tertiary Treatment. The inspector observed the two(2)ultra-screen disc filters. Both disc filters were
in operation and no issues of concern were observed with the tertiary treatment.
d) Disinfection. The facility disinfects the treated wastewater prior to discharge using ultra-violet light.
The facility does not use chlorine as a method of disinfection. No issues of concern were observed with
the disinfection system.
e) Solids Handling. The inspector observed the solids handling equipment(WAS pump station, One (1)
aerated sludge holding tank; Two(2) 3-belt filters presses with gravity thickening ability were
observed. No issues of concern were observed with the solids handling treatment units.
Hauler—The Town uses McGill Compost.
14. Effluent Flow is reported. The effluent flow meter is calibrated quarterly by YSI. The effluent flow meter
was last calibrated on January 11,2024. No issues of concern were observed with the flow measurement
device.
15. As required by the subject permit, influent and effluent sampling is achieved either by grab or composite
sampling. The effluent composite samplers are set up to collect flow-proportional samples over a 24-hour
period. The observed temperature for both the influent and effluent composite sampler refrigerators were
less than 5 degrees Celsius. The operator stated the tubing in the composite samplers is changed quarterly.
No issues of concern were observed with the composite samplers.
Note: The influent sampling station is located outside the WWTP facility's fencing footprint. However,
this influent sampling location has its own fence and access is secure and appropriate.
16. The facility has one emergency generator that will operate the entire WWTP. The operators test the
generators weekly and operate it under full load.
17. The inspector observed the effluent discharge from the outfall pipe to Terrible Creek to be clear with no
observable solids or foam. No observable detrimental impacts to the receiving waters from the effluent
discharge were observed.
NPDES Stormwater General Permit—NCG110000. Note: DWR staff did not conduct a General Stormwater
Permit inspection for the subject Treatment Works. The Division of Energy Mineral and Land Resources
(DEMLR)has oversight over this program element. If you should have questions regarding this permit,
coverage, or permit conditions components please do not hesitate to contact Raleigh Regional Office(919)791-
4200. You should request assistance form DEMLR stormwater staff on these matters. The below items/bullets
are for your reference.
• The general stormwater permit(NCG110000) applies to all owners or operators of stormwater point
source discharges associated with establishments primarily engaged in activities classified as Treatment
Works treating domestic sewage or any other sewage sludge or Wastewater Treatment Device or
System,used in the storage,treatment,recycling,and reclamation of municipal or domestic sewage
[including lands dedicated to the disposal of sewage sludge that are located within the confines of the
facility],with a design flow of 1.0 million gallons per day or more.

• Stormwater conditions includes but are not limited to the development and operations of Stormwater
Pollution Prevention Plan(SPPP). This plan should be fully developed, implemented, and updated
annually.
• Please be sure to conduct appropriate qualitative and analytical monitoring, as required by your permit.
• Detailed information that includes a list of all of the types Stormwater General Permits, including
NCG110000,Notice of Intent(permit application),DMR forms, qualitative monitoring forms can be
found and the NC DEMLR web site for stormwater. [click on Post construction stormwater. In the menu
on the right look for NPDES Industrial Program and then click on General Industrial Permits]
