Barry made reccomendations for improving erosion control measures at this location to prevent further sediment losses. The new multi-tiered basin was in place. page: 1 Site Number: 20120142 Owner: Thorpe, Gregory Inspection Date: 11/5/2015 Inspection Type: Site Inspection (DOT) Reason for Visit: Routine Unpermittabie Activities # Note: This section should be completed only if the impact is unpermittable (eg. Sediment, draining wetlands, etc.). Is stream(s) free of unpermittable impacts? Is wetland(s) free of unpermittable impacts? Is open water(s) free of unpermittable impacts? Are other standards being met (turbidity, pH, etc.) in all surface waters onsite? Are surFace waters on the site free of illegal discharges? Yes No NA NE ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ Comment: I met with Art King, Travis Morgan and Jason Crissman to review reported sediment losses near Sites 33 and 7. Barry Harrington was there as well to review erosion control measures throughout the project. The basin at Site 33 was overwhelmed resultinq in upwards of 10 cy of sediment discharged to a downstream wetland. Portions of the sediment had been cleaned but much remains in the wetland of which is currently too wet to access. DOT was directed to clean remaining sediment from wetland as soon as is practicable usinq whatever means required. This may include a pumper truck. Site 7 was cleaned of 57 stone satisfactorily. page: 2
