Dear Mr. Kirk:
A letter of request for an Authorization to Construct was received February 16,
1988, by the Division and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a 0.02 MGD wastewater treatment facility consisting of an influent
surge tank, a bar screen, a 25,000 gallon aeration basin, two (2) 175 CFM blowers., a
4,170 gallon clarifier, a 3,351 gallon sludge holding tank, a tablet chlorinator, a
520 gallon chlorine contact tank, a 1,000 gallon post aeration chamber, stand-by power
and a 35 GPM effluent pump station with high water alarms.
This Authorization to Construct is issued in accordance with Part III paragraph B
of the NPDES Permit No. NCO071528 issued December 17, 1987, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and -limitations'speci£ied in Permit No. NC0071528.
The Permittee must employ a certified wastewater operator in accordance with Part
III paragraph C of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
In event the facilities fail to perform satisfactorily in meeting its NPDES permit
effluent limits, the permittee shall take such immediate corrective action as may be
required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
This Authorization to Construct shall become voidable unless the Tri-Party
emN Agreement covering the operation and maintenance of this system between the Permittee
and -the North Carolina Division of Environmental Management is in full force and
effect.

The outfall line shall extend at least 500 ft. from the 760 elevation level in
order to maximize the dilution and dispersion of effluent.
Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional. engineer certifying
that the permitted facility has been installed in accordance with the approved plans
and specifications. Mai]. the certification to: Permits and Engineering Unit, Post
