Smith Utilities Compliance Manager/Pretreatment Coordinator City of Graham Narrative - 2023 City of Graham General Information General Program Information: * The NPDES Permit for City of Graham has an effective date of April 1, 2022 and an expiration date of May 31, 2026. * Sewer Use Ordinance and Enforcement Response Plan are up to date. * Long Term Monitoring Plan, Headworks Analysis (including allocation table) and Industrial Waste Survey were submitted to the NPDES Municipal Permitting Unit- Pretreatment office on Septen-iber 29, 2022 as required by our NPDES HNCO02121 I permit which had an effective date of April 1, 2022. Our office has not received approval of these items as of today, February 28, 2024. * Novaflex Hose Inc. and Stericycle Inc. permits were both renewed July 1, 2021 and will expire June 30, 2026. * Cintas Corporation's permit became effective on May 4, 2023 and will expire on June 30, 2026. This effective date was due to a modification of their permit because of the addition of a cleanroom processing area with process discharge and additional discharge pipe (Pipe 02) to the sanitary sewer. * Thunderbird Metals East's (formally Metals Impact East) permit became effective on June 28, 2023 and will expire June 30, 2026. This effective date was due to a modification of their permit due to a name update/change to more closely tie to their parent company. * USA Dutch Inc.'s permit was issued December 12, 2021 and will expire December 11, 2026. * City of Graham had no industries in Significant Noncompliance in 2023. * Data on the Division's Pretreatment Program Information Database was all correct, with the exception of the most current fiscal year PCI was done September 29, 2022 and the last Audit was done September 12, 2023, both by Tricia Lowery. Also, I attended the PAR workshop virtually on February 8, 2024. * Historical SNC Report as indicated by the NPDES Municipal Permitting Unit was not included this year for the PAR. For our facility our only SIU that would have been on the historical SNC report would have been Novaflex, IUPH 100 1, as having historical SNC for BOD. The time frame for their SNC was during the July 1, 2021 — December 31, 2021 time period. * All SlUs in our program were screened for 1,4-Dioxane during the July, 2023 — December, 2023 time period. Results are included in the IDSF forms for each respective industry. There were no immediate concerns. General Permit Information: SIU Permits for Cintas Corporation, Novaflex Hose Inc., Stericycle Inc., Thunderbird Metals East (formally Metals Irnpact East) and USA Dutcli Inc. are now in their third year of the five-year permit cycle. There were two permit modifications during 2023. Those are explained above under general program information. SIU Inforinatio 1. Cintas Corporation - Pipe 01 - Permit No. 0007 (Industrial Laundry) 640 Woody Drive, Graliam, N.C. 27253 Pipe 01 Cintas Corporation Pipe 0 1 had the following activities during the Jan. I - Jun. 3 0, 2023 reporting period: * Cintas Corporation Pipe 0 1 had no violations to report this period. * Orders and Schedule Information: None * A to C and Construction Information: None Cintas Corporation Pipe 01 had the following activities during the Jul. I - Dec.31, 2023 reporting period: * Cintas Corporation Pipe 0 1 had two TS S violation this reporting period for which Notices of Non -Compliance were issued. The Notices of Non - Compliance required 3 additional resamples. These resamples were taken by Cintas Corporation for the parameter and the resamples were within their permit limit. They were not in SNC for TSS. * Cintas Corporation Pipe 01 was inspected on October 16, 2023. * Orders and Schedule: None * A to C and Construction Information: None. Cintas Corporation - Pipe 02 Clean Room - Permit No. 0007 (Industrial Laundry) 610 Woody Drive, Graham, N.C. 27253 Pipe 02 Cintas Corporation Pipe 02 had the following activities during the Jan. I - Jun. 3 0, 2023 reporting period: • Cintas Corporation Pipe 02 had no violations to report this period. • Cintas Corporation Pipe 02 was inspected on June 7, 2024 • Orders and Schedule Information: None • A to C and Construction Information: Authorization to construct the wastewater storage pit, weir for discharge, ultrasonic flow meter and all associated valves, pumps, piping and appurtenances for the addition of the Pipe 02 discharge area for the clean room process was issued on March 28, 2023. Work was completed as specified and confirmed to be appropriate at the inspection on June 7, 2023. Cintas Corporation Pipe 02 had the following activities during the Jul. I - Dec.31, 2023 reporting period: • Cintas Corporation Pipe 02 had no violations to report this period. • Orders and Schedule: None • A to C and Construction Information: None 2 2. Novaflex Hose Corp. Permit No. 1001 40 CFR 428 (Rubber Manufacturing) 449 Trollingwood Road, Haw River, N.C. 27258 Novaflex had the following activities during the Jan. I - Jun. 3 ) 0, 2023 reporting period: * NovaFlex had no violations this reporting period to report. * Orders and Schedule Information: None * A to C and Construction Information: None NovaFlex had the following activities during the July I - Dec. 31, 2023 reporting period: * Novainex had one Copper violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required 3 additional resamples. These resamples were completed by Novaflex and the resamples were within their permit limit. They were not in SNC for Copper. * Novaflex had one Zinc violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required 3 additional resamples. These resamples were completed by Novaflex and the resamples were within their permit limit. They were not in SNC for Zinc. * Novaflex had two pH violations this reporting period for which a Notices of Non -Compliance were issued. The Notice of Non -Compliances required 2 additional resamples. These resamples were completed by Novaflex and the resamples were within their permit limit. * Novaflex was inspected on October 10, 2023. * Orders and Schedule Information: None 0 A to C and Construction Information: None 3. Stericycle, Inc. Permit No. 0030 (Medical Waste) 1168 Porter Ave., Haw River, N.C. 27258 Due to the extremely high levels of matrix interferences present in the Stericycle samples during samplings events during 2023, the commercial lab which was used for metals was unable to reach the lower detection limits for metals that are stated in the City of Graham LTMP during some sampling events. The commercial lab did provide documentation as to the cause of this. This documentation is kept with the sample results and is available for review if needed. Stericycle had the following activities during the Jan. I - Jun. 3 0, 2023 reporting period: Stericycle had one Zinc violation this reporting period for which a Notice of Non -Compliance was issued. The Notice of Non -Compliance required I additional resample. This resample was completed by Stericycle and the resample was within their permit limit. They were not in SNC for Zinc. Thunderbird Metals East (formally Metals Impact East) Permit No. 2001 40CFR 467.36 (Aluminum Forming) 235 Riverbend Rd., Graham, N.C. 27253 Thunderbird Metals East had the following activities during the Jail. I -Jun. 30, 2023 reporting period: * Thunderbird Metals East had no violations this reporting period. * Thunderbird Metals East was monitored for TTO compounds under 40 CFR 467. 36. Due to a high level or organic acids, esters and other non - target compounds present in the sample the laboratory was unable to achieve lower detection limits for volatile organic analysis. This sample required axl0 dilution.
