Complete if Approved: Rescind Immediately Reason for Approval: Mountaire removed sludge from the three unlined facultative lagoons at their process plant in Lumber Bridge in March 2004 and replaced the unlined lagoons with a 19.50 +/- million gallon lined, aerated lagoon. The sludge removal was a one-time event and is addressed by the annual report dated 9 June 2004 prepared by ECS Ltd. for Mountaire. The lagoon residuals where placed on crop land and corn was to be the receiving crop for the 2004 growing season. Based upon the visual observations made, on 26 October 2006, during a site visit to the Ammons farm fields that residuals where land applied to and discussions with Robert Jackson (ORC for Mountaire wwtp operations); the subject farm is in good condition with a current crop of soybeans awaiting harvesting. According to Mr. Jackson, Ammons farm yielded a typical corn crop for the 2004 growing season and with no unusual events or abnormal field/plant conditions that warranted investigation. Based upon the 2004 annual report, the PAN loading rate for corn on the fields utilized was well below that recommended or allowed for a corn crop.
