Please reference your Pre -Construction Notification (PCN) Form and attachments dated December 24, 2023,
for Department of Army (DA) authorization to discharge fill material into streams and other potential waters of
the United States to install a fiber optics cable along a 26 mile corridor. The approximate center of the
proposed project is located in directly west of North Front street and its intersection with Market Street, in
Smithfield, Johnston County, North Carolina.
Please also reference our notice (below) sent to you via e-mail on January 10, 2024, that your application was
incomplete and required additional information. This notice included requirements related to revisions to project
plans and the Pre -Construction Notification (PCN) Form, compliance with Nationwide Permit Regional and
General Conditions, and potential additional impacts to wetlands, to be submitted within 30 days of the date of
the request. To date we have not received all of the additional information necessary to proceed with your
request; therefore we now consider your application withdrawn. In order to allow us to process a DA
authorization request, please re -submit the PCN and relevant attachments, using guidance provided in our
notice dated January 10, 2024.
Please note that DA authorization must precede any discharge of dredged or fill material in waters of the US.

Good afternoon, my name is Matt Martin and I have been assigned as the project manager for your project. Thank you
for your PCN, dated December 24, 2023, for the above referenced project. I have reviewed the information and need
clarification before proceeding with verifying the use of Nationwide Permits (NWP) 57. Please submit the requested
information below (via e-mail is fine) within 30 days of receipt of this Notification. Please email
(matthew.k.martin@usace.army.mil) me if you have any questions/concerns or would like to set up at time to discuss.
General Condition 32(5) requires a delineation of wetlands, other special aquatic sites, and other waters, such as
lakes and ponds, and perennial and intermittent streams, on the project site. Wetland delineations must be
prepared in accordance with the current method required by the Corps. Please provide a delineation for the
complete project along the —25-mile alignment.
Please provide a cross section drawing similar to what was submitted for the West Market Street Crossing. This
Crossing will also require a Section 10 permit and Section 408 review.

General Condition 32(10) requires that the Corps can not make a permit decision until a review by the Corps
pursuant to 33 U.S.C. 408 because it will alter or temporarily or permanently occupy or use a U.S. Army Corps of
Engineers federally authorized civil works project. The Corps will coordinate this PCN with Civil Works once the
applicant provides an update for item 2 above.
