Attached is a revised PCN form A table was added to section C 31 that shows itemized impacts This table is also broken
down in more detail on the Impact drawings cover sheet that was previously submitted Bank stabilization is on the banks
only There is a note on both the initial plans and the plans for the extension that says what class of rip rap and "On Banks
Only" at each crossing
There are plans in the works to pave Gov Scott Farm Road NCDOT is currently preparing the plans and from what I
understand they plan to perform the work next spring I don't want to speak for the NCDOT on this matter, but they did
perform a survey on the road and I think the existing culverts were wide enough to accommodate their proposed work
without generating any additional impacts

Thank you for your PCN and attached information, dated and received 10/1/2015, for the above referenced project (SAW -
2013- 01456) I have reviewed the information and need clarification before proceeding with verifying the use of
Nationwide Permit 39 http / /www saw usace army mil/Portals /59 /docs/ regulatory/regdocs/NWP2012/NWP39_3 -23 pdf
Please submit the requested information below (via e -mail is fine) within 30 days of receipt of this Notification, otherwise
we may deny verification of the use of the Nationwide Permit or consider your application withdrawn and close the file
1) One small correction needed, is that the SG stream impacts should be itemized for fill due to culvert, fill due to bank
stabilization, and temporary impacts, all of this should show in section C 3 of the PCN If nothing else, this can be
clarified in the notes (C 31) Also, is the bank stabilization just rock on the banks, or does it include rock in the channel?
If rock in the channel, will the rip rap pad include rock sitting on top of the stream bed or (our preference) rock installed
so that the top of the rock is no higher than the original stream bed elevation?
2) Lastly, Senator Ralph Scott Parkway is now proposed to extend to Gov Scott Farm Road. The PCN mentions traffic
volumes including truck traffic, which brings into question the potential need to improve /pave Gov Scott Farm Road.
Does NCDOT or another entity plan to make these improvements? If so, what is the expected timing on such work? Note
