Hey Brian,
Attached is the revised page 3 for the U-3613B application. In
the table for stream impacts, site 2 was changed to site 3 to
match the buffer impact table. Site 2 is an existing site with
no culvert extension or replacement. However, the road is being
widened and that is the reason for the buffer impacts. Let me
know if you have any more questions.
