CC T0: USACOE - Mr. John Thomas Signed: Alex T. Carter, PE BEESOR 8 CARTER, PR. CIVIL ENGINEERS LAND SURVEYORS LAND PLANNING October 9, 2015 Mr. John Thomas Raleigh Regulatory Field Office US Army Corps of Engineers 3331 Heritage Trade Drive - Suite 105 Wake Forest, NC 27587 OCT g 4 2015 RE: PCN To Modify Impacts submitted under SAW - 2014 -01724 and DWR 2014 -0972 Mr. Thomas — The PCN submitted September 11, 2014 and 404 Certification issued September 19, 2014 is requested for modification. The previous PCN requested two culvert crossing with stabilization. The larger of those culvert will not be installed due to costs involved. There has been a utility (sewer) connection installed along with bank stabilization and a headwall repair just upstream on an existing culvert. As part of that repair, stabilization was installed prior to requesting the modification. I have attached pictures of the stabilization installed. The net impact of the requested modifications is: • NO permanent culvert crossing and stabilization installed - Noted as S2 on prior PCN; I OLF crossing, 50LF stabilization • ADD Installation of stabilization at a location 200' upstream for an existing culvert (Noted as S5 this modification request PCN; 25LF) • MODIFY Installation of stabilization along bank for utility crossing —Noted as S2 on prior PCN; 15LF stabilization left side only looking downstream • NET impact is 20LF less permanent impact I have met with Sue Homewood on site September 24, 2015 to review the impacts based on a neighbor's complaint. I would further note a concern regarding the level of the stone stabilization installed at the existing culvert. The stream bed had scoured down several feet in this area and the stone placed returned the bed to the invert of the culvert. The water flow has been below the level of the recently installed rock stabilization. The significant rainfall over the past 7 -10 days (end of September) has begun to fill the voids and re- establish the bed closer to the pipe invert and water is now visible in the newly stabilized area.
