Rescission requests
I have three SFR facilities that no longer need a discharge permit. Two have had conventional septic systems installed
and approved by the County Health Departments and one never had a system installed. I have already emailed
instructions for the rescission letter and plan to mail them copies of the inspection reports with instructions as well. I
have not heard back from the owners, but I wanted to let you know there may be three more we can remove from
the SFR list. They are permit numbers NCG550457, NCG550304 and NCG550350. NCG550304 has had new owners
since 2006 and NCG550350 was combined with another plat in 2003. Please let me know if you need anything specific
on my end.

Mikal — I got lost in the highlighted portions of your message [see below]. Here's what I know about the two sites:
NCG550348
Original wasteload allocation (1985) and original AtC (1986) reference permit NCO062596 for Lot 216.
Original permit issued to Roy Heussner.
ARO staff report from 1991 references 216 Summit Drive.
Our file has no CoC copies, so I don't know when the last paper CoC was issued. I'm not sure a renewal was ever issued
to Mr. Heussner.
CHW attempted to contact owner Heussner in 2004, was unsuccessful.
ARO inspection in 2006 could not locate sandfilter.
CoC was automatically renewed because no unpaid fees appeared in BIMS. CoC is set to No Fee, so no invoices are
sent. No annual fees have ever been paid.
Haywood County GIS has no info for 216 Summit Drive. That address does not appear in their records.

I think the facility mixup occurred because Roy & Leona Heussner were the original permittees. When Leona's parcel
was inspected in 1991, it was mistakenly assigned Roy's address. I later entered that change into BIMS.
NCG550348 should be assigned to 552 Summit Drive.
NCG550350 should be assigned to 149 Overlook Drive.
I'll correct BIMS today.
Both properties have new owners, and both should submit name & ownership change forms with their deeds. Proceed
as you see fit to inspect 149 Overlook Drive and contact the current owners of 552 Summit Drive.

Sorry for all the emails about these SFRs, but I have more questions as I'm going through the files. The two I am looking
at appear to have been switched at some point over the years.
Permit NCG550348 was at one time NC0062696 and covered Lot #216.
NCG550350 was originally NC0062626 and covered Lot 4228 of the Mountain Top Development in Maggie Valley.
On Haywood County GIS Lot #216 is now 552 Summit Dr. and Lot #228 is 149 Overlook Dr.
I believe at some point Lot #216 ended up with the permit for Lot #228 since the lots were under one developer in the
past. Could you verify for me if what I've found is correct?
I'm only asking because 552 Summit Dr. (Lot #216) was inspected last year under permit #NCG550350; however, Lot
#216 (NCG550348) is due for an inspection this year.
Do we need to correct the addresses for these two permits? 149 Overlook Dr (Lot #228) still has not been inspected
because of the switch.

On March 26, 2015, Andrew Moore of the Asheville Regional Office conducted a compliance evaluation
inspection of the Exum residence. Mr. Exum was notified prior to the inspection, but was not present
as the residence is a second home. No infrastructure associated with the discharge system was
located during the inspection. A PVC wastewater pipe was observed exiting the home at the northern
building foundation. However, no additional infrastructure associated with the system was located. At a
minimum, the permittee should locate, and maintain access to, the effluent discharge pipe. In addition,
it is recommended that the septic tank be pumped every 3-5 years for full-time residences. The
permittee should use his discretion in determining when to pump the septic tank, depending on how
often the home is inhabited.
The residence is currently for sale. If the residence is sold, the permittee should notify the new owners
of their requirement to complete a Change of Ownership form. They may contact the Asheville
Regional Office for additional guidance.

A letter of request for Authorization to Construct was received April 10, 1985,
by the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a 300 GPD wastewater treatment facility consisting -of a 1, 000 gallon
septic tank, a flow distribution box, and a standard subsurface sandfilter measuring
6 feet by 43 feet with a discharge into Mine Branch.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NCO062626 issued October 1, 1985, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0062626.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Asheville Regional Office, telephone number 704-253-3341 shall be
notified at least twenty-four (24) hours in'advance of backfilling of the installed
subsurface filter system so that an in -place inspection can be made of said system
prior to backfilling. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through
Friday, excluding State Holidays.

may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
The sand media of the sub -surface filter must comply with the Division's sand
specifications and must be analyzed and approved by this Division either by direct
sampling or by acquisition of filter sand from a dealer who is currently certified
by the Division as an acceptable source.
