Below is the Pretreatment Annual Report Narrative for the reporting period January 1,
2023 to December 31, 2023.
GENERAL INFORMATION:
DENTAL AMALGAM RULE:
• No new information to report.
GENERAL PROGRAM INFORMATION:
• AT, LTMP, SUO, ERP, IWS, and Permits are up to date.
• HWA was submitted 4/23/21but has not received Unit approval.
• LTMP was updated and approved by the Unit on 12/15/21.
• IWS was submitted l/10/2024 but has not received Unit approval.
• SUO was approved by the Unit on 10/25/12.
• ERP was approved by the Unit on 2/5/20.
• Surry County Landfill was issued a local IUP effective 7/1/21.
• Prime Kleen Commercial Laundry was issued a local IUP effective 3/15/23.
• ITW Texwipe was issued an IUP effective 7/l/22. The permit and updated AT
was approved by the Unit 5/20/22.
• Prime Kleen Commercial Laundry was issued an IUP effective 3/l/24. The permit
and updated AT has not received Unit approval.
• Dates on the Division's Database Program Information Sheet are correct. The
Program Information Sheet is included.
• A copy of our PAR delegation letter is provided.

To whom it may concern,
Section 1.1 and 1.2 (a) (25) of the City of Mount Airy's Sewer Use Ordinance designates
our Director of Public Works as the POTW Director and delegates responsibility for
overall operation of the City of Mount Airy's Pretreatment Program. It also states that any
powers granted to or imposed upon the POTW Director may be delegated by the POTW
Director to other City personnel. Therefore, I hereby designate the City's Wastewater
Treatment Plant Supervisor with the authority to sign all required documents under the
Pretreatment Program, including the authority to sign the Pretreatment Annual Report
(PAR) and submit it to NPDES Wastewater Permitting Branch. Currently, the City of
Mount Airy's Wastewater Treatment Plant Supervisor is Mr. Christopher Marion.
