General Information for 2023: Pretreatment Program Info Database sheet sent for 2023 is in need
of update in regards to PT contact information. Updated contact information was sent in the 2022
PAR aswell as noted in the 2023 PCI and again is corrected and enclosed for your records.

regards to Pretreatment Program Info Database have been made and reflected on enclosed PID

The only SIU permitted for the Cooleemee WWTP is the Ingersoll Rand facility (IUP 004). The
permit was renewed on 1/1/21 and expires on 1/1/25
Semi-annual sampling reports indicate that the SIU is well within the permitted limits for all
constituents.

Remarks
On June 27, 2023 Tricia Lowery of the Winston-Salem Regional Office conducted a routine
pretreatment audit inspection on the pretreatment program. Michael Weir made two inquiries in
regards to the SUO and HWA. It was noted for the SUO that it was updated on November 6, 2017
and submitted to the State but no approval letter was found on file. Inquiries were made as to if an
approval letter was sent to Davie County since none on file. The second inquire was made in
regards to our HWA. All records show HWA was received by DWR on 9/28/2021 but County has
received no notification of approval of the HWA or shown as approved on the most recent
Pretreatment Program Info Database. This was brought to the attention of Alex Lowe during the
August 1, 2022 pretreatment audit inspection and again on 6/27/2023 to Tricia Lowery during the
PCI.
