Mr. Thomas:
We need to request a permit modification on the Winston Salem Nor[hern Beltway (U2579B) in
Forsyth County. On Structure # 10 that is currently being constructed, there appeazs to be a spring
with a fair amount of water flow around the bridge pile that was driven on top of the spring. In
order for the remaining spread footer construction to be completed and to alleviate the water flow
in this area, we would like to request permission to allow the contractor to install a one foot
French drain running from the comer of the spread footing to the flowline of Smith Creek
approximately 100 feet downstream (see attachment for location). The French drain would
consist of 1 foot of #57 washed stone encapsulated in Geotextile fabric. The outlet end of the
French drain would consist of a 6" PVC outlet pipe. The work will require approximately 10' of
temporary impacts to Smith Creek. The wark will involve excavating the bank of Smith Creek,
laying the outlet pipe, backfilling the creek bank, spreading a native seed mix and covering with
coir fiber matting. The work will also impact less than 0.01 acres to the spring. The work area
will be isolated from flowing water.
