sign if necessary.

1. For Field NC-UN-035-02, please verify the location and usage (public water or irrigation) of any
existing wells and their appropriate setbacks. Maria Schutte of the Mooresville Regional Office
(MRO) commented that this field may be similar to Field NC-UN-035-03, which indicates a
habitable residence with a well. [ 15A NCAC 02T .1108(c)]
2. For Field NC-UN-035-03, Maria Schutte of the MRO commented that the acreage on the southwest
side of the pond will not be used for land application. Please verify the land application use and
revise the applicable maps for acreage changes.

reflect the most recent upgrades (centrifuge, residual processing, etc.) described by the MRO.
Please provide an updated process flow diagram. This will not hold up the permit renewal process.
