There was a question about what permit package the core received as the permit package available on the NCDOT
Connect site (dated 20190523) shows all of what the reviewer has commented on and is asking to show.

First, I need to know the dimensions of the existing culverts at Sites #1 & #3, specifically, total culvert lengths. Site 1-
36"RCP length= 48'; Site 2- 3@103"x71" CMAP length= 60'. 1 cannot tell from the permit drawings if the impact totals
provided on the Impact Summary Sheet include the total length of impacts in the jurisdictional waters from the project
with consideration (subtraction) of the lengths of the existing culverts or not. This is important because the NWP
authorization would be for impacts to jurisdictional waters beyond the existing structures and likewise the mitigation
required for your project should include the impacted jurisdictional features beyond the existing structure. Existing
transportation facilities (ETF) was not in included in the impacts. Impacts only include those areas beyond the existing
structures. (See snips below for clarification). In addition, the site plans indicate at Site #3 that the project will include
35 linear feet (inlet end) and 40 linear feet (outlet end) of channel improvement. I cannot tell from the application or
drawings what the channel improvement will include. Is this channel realignment, sediment removal, bank stabilization,
channel armor, etc.? If this channel improvement will include relocation, excavation/fill and/or placement of rock in the
channel, these activities should be specified, and the permit plans should include a typical or actual cross -sectional view
of what these improvements will be. Yes, Channel improvements include all of what is mentioned and shown in the
cross-section detail at the bottom of PSH 6 (cross section from plans shown below). The project Plans provided to the
Corps should clearly show both the existing and proposed structures with call outs for impact areas and the
activities/structures that are impacting these waters. The provided drawings are a bit busy and do not clearly show/call
out the impacts. Channel improvements description and cross -sectional view is shown at the bottom of the permit PSH 6
and a zoomed in view of the sites are shown on PSH 6A. Finally, once it is determined/quantified what impacts to
jurisdictional waters this project will have, NCDOT should propose mitigation for the waters that will be lost from the
impacts.

As a follow up to our conversation today I wanted to send you an e-mail and briefly outline the information that I need
for the permit application for NCDOT U-5711, Pineview Street in Randolph County.
First, I need to know the dimensions of the existing culverts at Sites #1 & #3, specifically, total culvert lengths. I cannot
tell from the permit drawings if the impact totals provided on the Impact Summary Sheet include the total length of
impacts in the jurisdictional waters from the project with consideration (subtraction) of the lengths of the existing

Also, since the application included a mussel survey dated November 2023 that recommended a may affect but not
likely to adversely affect the Atlantic Pigtoe, I will need to coordinate/consult with USFWS to ask for their concurrence
