15A NCAC 02T .0400 — SYSTEM -WIDE WASTEWATER COLLECTION SYSTEMS
INSTRUCTIONS FOR FORM CSA 04-16 & SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.
A. Cover Letter
® Submit a cover letter listing all items and attachments included in the permit application package
B. No Application Fee Required
➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit
➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at:

C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application:
® Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM: CSA 04-
16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.
❑ If the Applicant Type in Section I.3 is a Privately -Owned Public Utility, provide the Certificate of Public
Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or
❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an
application for a franchise has been received and that the service area is contiguous to an existing franchised area
or that franchise approval is expected.
❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for
business with the North Carolina Secretary of State.

➢ The Authorized signing official listed in Section I.4 should match with that of the Applicant certification page in
accordance with 15A NCAC 02T .0106(b). Per 15A NCAC 02T .0106(c), an alternate person may be designated
as the signing official if a delegation letter is provided from a person who meets the criteria in 15A NCAC 02T
.0106(b).
➢ NOTE - Public Works Directors are not authorized to sign this permit application, according to the rule,
unless they are formally delegated.

A RESOLUTION ADOPTED BY THE BOARD OF COMMISSIONERS
OF THE TOWN OF FUQUAY-VARINA, NC
TO ADOPT AND APPROVE THE 2023 PUBLIC UTILITIES CAPTIAL IMPROVEMENT PLAN
WHEREAS, The Town of Fuquay-Varina has developed a water and wastewater system in and around
the Town of Fuquay-Varina and Wake County; and
WHEREAS, Said municipal water and wastewater systems are operated and maintained by the Town of
Fuquay-Varina; and
WHEREAS, the Town of Fuquay-Varina recognizes the need for, and advantage of maintaining adequate
short-term and long-term planning for capital improvements for the Town's water and
wastewater systems.
NOW, THEREFORE BE IT RESOLVED, by the Board of Commissioners of the Town of Fuquay-Varina, North
Carolina, the following:
1. The Town of Fuquay-Varina that the 2023 Public Utilities Capital Improvement Plan for the water and
wastewater system is hereby adopted and approved; and
2. BE IT FURTHER RESOLVED that the Town Board authorizes staff to update the plan as new
information becomes available.
This Resolution of the Town of Fuquay-Varina Board of Commissioners is hereby adopted this 191' day of
September in the year 2023 in Fuquay-Varina, North Carolina.

Repair bands are stored in the sewer collection and water distribution work bays. 2-inch
submersible bypass pump with hose, fittings, and control box are stored under the
covered shelter at the public service center.
Sewer Spill Response Plan
1. Police department dispatchers will contact primary on -call person. Primary on -call
person will make an on -site assessment of the situation within 60 minutes of having
been contacted by police dispatch.
2. Primary on -call person will contact additional staff as needed or direct police
dispatch to do so.
3. The primary on -call person will document time, location, and equipment used.
2 HOW TO RESPOND
Sewer spill on concrete or asphalt
1. Setup containment at or near storm drainage system; do not let spill enter storm
drainage system.
2. Spread lime and sand on spillage. Let this stand for 1 hour and sweep up. If
moisture or grease is still present, repeat the procedure.
3. After removing contaminated material, wash down the area with high waterpressure
and bleach.
4. Transport contaminated material to Kenneth Branch treatment plant and place on
drying bed. Allow material to dry and then contactwaste haulerto dispose of
material.
• Sewer spill in streams
1. Take initial assessment dissolved oxygen readings in the stream, upstream and
downstream of point of entry. Determine point of impact limit on downstream by
monitoring for dissolved oxygen levels. The point downstream where dissolved
oxygen levels are at the lowest and begin to gradually increase should be, if
possible, the location of where the dam is to be built.
2. The dam should be constructed to handle the existing stream flow in addition to
whatwater is added to properly flush the stream.

3. The dam should be monitored constantly to ensure it does not fail. Repair material
and manpower should be on -site to make immediate repairs if needed.
4. Accumulated water and sewer at the dam site should be pumped directly to the
nearest manhole.
5. Flushing of the stream, monitoring fordissolved oxygen levels, and running
bacteriological analysis should continue until levels are reached thatare
comparable if not better than the levels upstream of the entry point.
The response personnel are not to be limited by the procedures listed above.

procedures which the situation may require.

1 HOW TO RESPOND
Procedures for responding to sewer pump failures shall consist of but not be limited to
the follow procedures...
1) Failed pumps shall be pulled immediately, and notice given to the contract pump
repair shop so arrangements for pick-up can be made.
2) Pump repair shop will provide the Town of Fuquay-Varina with a proposal to
repair versus replace the failed pump.
3) Collections Supervisor and Operations Manager will collaborate on deciding
whether to repair or replace the failed pump.
4) Purchase Requisition will be completed forthe option chosen and submitted to
the Public Utilities Director for approval and then submitted to Finance forfinal
verification of funds availability and issuance of purchase order number.
5) The purchase order numberwill be given to pump repair contractor so work can
begin.

Should a catastrophic incident such as control panel failure, multiple pump failure, or
force main failure occur at a sewer pump station emergency pump and haul and/or
pump around procedures shall be implemented. If a bypass connection exists, the
towns 4" and 3' will be put into place. If further pump around is needed, then the
contacts below will be utilized until the problem can be corrected.

3 EMERGENCY EQUIPMENT AND SPARE PARTS
The following equipment and materials are located at the Public Service Center
o Jet -Vac Truck
o 3-inch bypass pump and hose are in the spill response trailer
o 4-inch bypass pump is stored at TC WWTP in the maintenance shop
o 4-inch bypass hose and fittings are in the spill response trailer

Contingency Scope
This plan includes all data currently on hand for each of the lift stations under town of
Fuquay-Varina Jurisdiction. Each lift station has a duty point in Gallons per minute and
feet of Total dynamic head, Pump selection, and diameter of suction and discharge pipe
needed to bypass. Please upsize the diameter of hoses & accessories on the
equipment list to correspond with the required diameter on the data page. The pump
model section is color coded green for lift stations that the town has the capability to
bypass, and orange if the current town fleet does not have the capability.
