Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System -wide Collection System Permitting website: General — When submitting an application to the Municipal Permitting Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one copy of the application and supporting documentation. The copy may be submitted in digital format. A. Cover Letter ® Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit ➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at: ➢ Annual Non -Discharge Fees C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application: ® Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM: CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section I.3 is a Privately -Owned Public Utility, provide the Certificate of Public Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State. Sewer Spill Response Plan 1. Police department dispatchers will contact primary on -call person. Primary on -call person will make an on -site assessment of the situation within 60 minutes of having been contacted by police dispatch. 2. Primary on -call person will contact additional staff as needed or direct police dispatch to do so. 3. The primary on -call person will document time, location, and equipment used. 2 HOW TO RESPOND Sewer spill on concrete or asphalt 1. Setup containment at or near storm drainage system; do not let spill enter storm drainage system. 2. Spread lime and sand on spillage. Let this stand for 1 hour and sweep up. If moisture or grease is still present, repeat the procedure. 3. After removing contaminated material, wash down the area with high waterpressure and bleach. 4. Transport contaminated material to Kenneth Branch treatment plant and place on drying bed. Allow material to dry and then contactwaste haulerto dispose of material. • Sewer spill in streams 1. Take initial assessment dissolved oxygen readings in the stream, upstream and downstream of point of entry. Determine point of impact limit on downstream by monitoring for dissolved oxygen levels. The point downstream where dissolved oxygen levels are at the lowest and begin to gradually increase should be, if possible, the location of where the dam is to be built. 2. The dam should be constructed to handle the existing stream flow in addition to whatwater is added to properly flush the stream. Page 3of5 3. The dam should be monitored constantly to ensure it does not fail. Repair material and manpower should be on -site to make immediate repairs if needed. 4. Accumulated water and sewer at the dam site should be pumped directly to the nearest manhole. 5. Flushing of the stream, monitoring fordissolved oxygen levels, and running bacteriological analysis should continue until levels are reached thatare comparable if not better than the levels upstream of the entry point. The response personnel are not to be limited by the procedures listed above. Response personnel should use the procedures listed above in addition to any other procedures which the situation may require. If a bypass connection exists, the towns 4" and 3' will be put into place. If further pump around is needed, then the contacts below will be utilized until the problem can be corrected.
