Resources (the Division) has reviewed your request to remit civil penalties in the subject case. We regret
to inform you that the Director has denied your request.
Our records indicate the facility received the penalty letter on August 31, 2015. A response by either
payment, 'a written request for remission, or a petition for an administrative hearing with the Office of
Administrative Hearings was due by September 30, 2015. The remission request was dated and signed
on October 2, 2015, which is outside of the 30 day statutory window.
Therefore, your outstanding balance ($1,643.85) is due and payable.
