This permit, issued on February 19, 2024 , is subject to compliance with the application (where
consistent with the permit), all applicable regulations, special conditions and notes set forth below. Any violation
of these terms may be subject to civil or criminal penalties; or may cause the permit to be null and void.
NOTE: This Transfer/Renewal must be attached to copies of the following permit documents, which
includes all application forms and workplan drawings:
a. The New Permit 21-90 issued on 2/14/90,
b. the renewals issued on 9/10/93, 3/15/99, 3/16/09, 1/24/19
c. the transfer/renewal issued on 9/24/14
d. the minor modification/renewal issued on 10/29/13
e. the minor modification issued on 11/24/03,
f. the major modification issued on 6/5/15

1) Copies of all referenced documents shall be readily available on site when a Division representative
inspects the project for compliance. In accepting this transfer, the new permittee agrees that all
conditions and stipulations of the active permit remain in force. \[07J .0406\]

This permit action may be appealed by the permittee or other Signed by the authority of the Secretary of DEQ and the Chair
qualified persons within twenty (20) days of the issuing date. of the Coastal Resources Commission.

This permit must be accessible on-site to Department

personnel when the project is inspected for compliance.
