Guidelines for Access: The staff of the Raleigh Regional Office is dedicated to making public
records in our custody readily available to the public for review and copying. We also have the
responsibility to the public to safeguard these records and to carry out our day -today program
obligations. Please read carefully the following before signing the form.
1. Due to the large public demand for file access, we request that you call at least a day in
advance to schedule an appointment for file review so you can be accommodated.
Appointments are scheduled between 9:00 a.m. and 3.00 p.m. Monday -Friday Viewing time
ends at 4:00 p.m. Anyone arriving without an appointment may view the files to the extent that
time and staff supervision are available.
2. You must specify files you want to review by facility name or incident number, as appropriate.
The number of files that you may review at one appointment will be limited to five.
3. You may make copies of a file when the copier is not in use by the staff and if time permits.
Cost per copy is 2.5 cents for ALL copies if making more than 25 copies - there is no charge if
less than 25 copies are made. Payment is to be made by check, money order, or cash in the
administrative offices.
4. FILES MUST BE KEPT IN THE ORDER YOU RECEIVED THEM. Files may not be taken
from the office. No briefcases large totes etc are permitted in the file review area. To
remove, alter, deface, mutilate, or destroy material in one of these files is a misdemeanor for
which you can be fined up to $500.00.
5. In accordance with General Statute 25-3-512, a $25.00 processing fee will be charged and
collected for checks on which payment has been refused.
6. The customer must present a photo ID, sign -in, and receive a visitor sticker prior to reviewing
files.

Dear Mr. Collins:
Your letter received on January 19, 2012 concerning corrective actions for your
analytical procedures have been reviewed. The quality control measures, taken
in reference to the findings cited in the December 8, 2011 inspection report are
acceptable, with exception of the following comment:
Ammonia Nitrogen - Standard Methods, 18th Edition, 4500 NH3 C (Nesslerization)
B. Finding: The laboratory is not analyzing matrix spikes.
Requirement: Unless the referenced method states a greater frequency, spike 5% of
samples on a monthly basis. Laboratories analyzing less than 20 samples per month
must analyze at least one matrix spike each month samples are analyzed. Prepare the
matrix spike from a reference source different from that used for calibration unless
otherwise stated in the method. If matrix spike results are out of control, the results must
be qualified or the laboratory must take corrective action to rectify the effect, use another
method, or employ the method of standard additions. When the method of choice
specifies matrix spike performance acceptance criteria for accuracy, and the laboratory
chooses to develop statistically valid, laboratory -specific limits, the laboratory -generated
limits cannot be less stringent than the criteria stated in the approved method. Ref: North
Carolina Wastewater/Groundwater Laboratory Certification Policy.
Comment: A copy of the matrix spiking policy was given to the laboratory during the
inspection.
Recommendation: It is recommended that the spike solution constitutes :51% of the
total MS volume.

Per your e-mail of October 21, 2011 you were working on some calculations and
acceptable scheme that I can work with.
Auditor Response: it is recommended that the laboratory followup Option-1 of the
laboratory "Matrix Spiking Policy". The amount of spiking solution to be used is based on
the volume of sample being used and the concentration of the sample being spiked. A
copy of the matrix spiking policy was left with the laboratory at the time of the inspection.
No further response is necessary.
Please continue to follow all approved methods, rules and regulations.

Approval of the Plans and Specifications

The review for completeness and adequacy of the project construction plans and
specifications has been concluded by the Construction Grants & Loans Section of the North
Carolina Division of Water Quality. Therefore, said plan documents are hereby approved.
Eligibility for Revolving Loan funding is determined as follows:
Eligible
Three (3) 250 gpm aspirators; two (2) 500 gpm sludge recirculation pumps and one pump motor;
425 feet of ductile iron pipe; and three (3) 2.7 Hp mixers.
Issuance of this approval letter does not imply availability of funding. Please note that the
Clean Water State Revolving Fund (CWSRF) program is priority based system which includes
adhering to a schedule for submittal of applications through contracting. Therefore, in order to
maintain assurance of funding an Authority to Award must be obtained by December 1, 2012
and all contracts must be executed by January 2, 2013.
If any of these milestones are not met, you must reapply if you wish to seek CWSRF
funding for this project. The CWSRF program accepts applications twice per year and projects
are funded based on priority and availability of funds. We encourage you to try to complete
these milestones well before the due date to build in contingency time.
All encroachments, easements, agreements, and plan approvals must be obtained prior to
receiving Authority to Award.

In the event that received bids exceed the amount established through the funding offer,
and local funds are not adequate to award contract(s), it will be necessary to consider all
alternatives including redesign, re -advertising, and rebidding.
Neither the State nor Federal Government, nor any of its departments, agencies or
employees is or will be a party to the invitation to bids, addenda, any resulting contracts or
contract negotiations/changes.
If the Recipient does not maintain the court -sanctioned schedules which extended the
compliance date for complying with the final effluent limits established in the NPDES Permit,
then project costs incurred will not be eligible for Revolving Loan payments.
Your project is subject to the one-year performance certification requirements. By this,
you are required on the date one year after the completion of construction and initial operation of
the subject treatment facilities, to certify, based on your consulting engineer's advisement,
whether or not such treatment works meet the design performance, specifications and the permit
conditions and effluent limitations.
In accordance with the Federal Regulations, the Recipient is required to assure compliance
with the OSHA safety regulations on the subject project. In complying with this regulatory
responsibility, the Recipient should, by letter, invite the Bureau Chief, Education Training and
Technical Assistance Unit, NC OSHA Division, 1101 Mail Service Center, Raleigh, NC 27699-
1101 at (919) 807-2890, to participate in the Preconstruction Conference to assure that proper
emphasis is given on understanding and adhering to the OSHA regulations.
It is the responsibility of the Recipient and the Consulting Engineer to insure that the
project plan documents are in compliance with Amended N. C. G. S. 133-3 (ratified July 13,
1993). The administrative review and approval of these plans and specifications, and any
subsequent addenda or change order, do not imply approval of a restrictive specification for
bidding purposes; nor is it an authorization for noncompetitive procurement actions.
Any addenda to be issued for subject project plans and specifications must be
submitted by the Recipient such that adequate time is allowed for review/approval action
by the State and for subseauent bidder action prior to receipt of bids.
It is mandatory for project facilities to be constructed in accordance with the North
Carolina Sedimentation Pollution Control Act, and, when applicable, the North Carolina Dam
Safety Act. In addition, the specifications must clearly state what the contractors' responsibilities
shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable project Permits from the State, including an Authorization to Construct and/or Non -
Discharge Permit.

While rejection of all bids is possible, such action may be taken only with prior State
concurrence, and only for good cause.
A goal of 8% of the contract price is established for Minority Business Enterprise (MBE)
participation in this project, and a goal of 5% of the contract price is established for Women's
Business Enterprise (WBE) participation in this project. The Recipient and Bidders shall make a
good -faith effort to assure that MBE's and WBE's are utilized, when possible, as sources of
goods and services. The good -faith effort must include the following affirmative steps: (a)
including small, minority, and women's businesses on solicitation lists; (b) assuring that small,
minority, and women's businesses are solicited whenever they are potential sources; (c) dividing
total requirements, when economically feasible, into small tasks or quantities to permit
maximum participation by small, minority, and women's businesses; (d) establishing delivery
schedules, and (e) using the services of the Small Business Administration and the Minority
Business Development Agency of the U.S. Department of Commerce. Please note that the
solicitation efforts should include documentable follow-up phone calls.
The Recipient shall comply with the provisions of 40 CFR, Part 7, Subpart C -
Discrimination Prohibited on the Basis of Handicap.
Attached is one (1) copy of the Project Bid Information which is to be completed within 21
days after bids have been received, and submitted to the State for review. Upon review and
approval of this information, the State will authorize the Recipient to make the proposed award.
Do not proceed with construction until the Authority to Award package and the EEO and
MBE documentation/certification have been reviewed, and you are in receipt of our approval, if
a CWSRF loan is desired for project construction.
Two (2) copies of any change order must be promptly submitted by the Recipient to the
State. If additional information is requested by the State, a response is required within two (2)
weeks, or the change order will be returned without further or final action.
One (1) set of the final approved plans and specifications will be forwarded to you. One
(1) set of plans and specifications identical to the approved set must be available at the project
site at all times.
Upon completion of the project construction, the Recipient shall submit a letter confirming
that the project has been constructed in accordance with the plans and specifications approved by
the State. "As -built" plans will need to be submitted with any changes clearly documented on
the plans if the above confirmation cannot be made.

Dear Mr. Warren:
A letter of request for an Authorization to Construct was received October 17, 2011, by
the Division of Water Quality (Division), and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of modifications to the existing 1.37 MGD Wastewater Treatment Plant, with
discharge of treated wastewater into the Tar River in the Tar -Pamlico River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Replacement of three (3) existing aerators with three (3) new 250 gpm aspirators;
replacement of two (2) 500 gpm sludge recirculation pumps; and replacement of
three (3) 2.7 Hp anaerobic selector zone basin mixers, in conformity with the
project plans, specifications, and other supporting data subsequently filed and
approved by the Department of Environment and Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO020231 issued February 17, 2010, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in Permit No. NCO020231.
The sludge generated from these treatment facilities must be disposed of in accordance
with G.S. 143-215.1 and in a manner approved by the Division.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may

be required by the Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to: Construction Grants & Loans,
DWQ/DENR, 1633 Mail Service Center, Raleigh, NC 27699-1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at
least equivalent to or greater than the classification assigned to the wastewater treatment
facilities by the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.

Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

be required by the Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility
has been installed in accordance with the NPDES Permit, this Authorization to Construct and the
approved plans and specifications. Mail the Certification to: Construction Grants & Loans,
DWQ/DENR, 1633 Mail Service Center, Raleigh, NC 27699-1633.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at
least equivalent to or greater than the classification assigned to the wastewater treatment
facilities by the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class H, III and IV facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.

Failure to abide by the requirements contained in this Authorization to Construct may
subject the Pemuttee to an enforcement action by the Division in accordance with North
Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

The review for completeness and adequacy of the project construction plans and
specifications has been concluded by the Construction Grants & Loans Section of the North
Carolina Division of Water Quality. Therefore, said plan documents are hereby approved.
Eligibility for Revolving Loan funding is determined as follows:
Eligible
Three (3) 250 gpm aspirators; two (2) 500 gpm sludge recirculation pumps and one pump motor;
425 feet of ductile iron pipe; and three (3) 2.7 Hp mixers.
Issuance of this approval letter does not imply availability of funding. Please note that the
Clean Water State Revolving Fund (CWSRF) program is priority based system which includes
adhering to a schedule for submittal of applications through contracting. Therefore, in order to
maintain assurance of fundine an Authority to Award must be obtained by December 1, 2012
and all contracts must be executed by January 2, 2013.
If any of these milestones are not met, you must reapply if you wish to seek CWSRF
funding for this project. The CWSRF program accepts applications twice per year and projects
are funded based on priority and availability of funds. We encourage you to try to complete
these milestones well before the due date to build in contingency time.
All encroachments, easements, agreements, and plan approvals must be obtained prior to
receiving Authority to Award.

In the event that received bids exceed the amount established through the funding offer,
and local funds are not adequate to award contract(s), it will be necessary to consider all
alternatives including redesign, re -advertising, and rebidding.
Neither the State nor Federal Government, nor any of its departments, agencies or
employees is or will be a party to the invitation to bids, addenda, any resulting contracts or
contract negotiations/changes.
If the Recipient does not maintain the court -sanctioned schedules which extended the
compliance date for complying with the final effluent limits established in the NPDES Permit,
then project costs incurred will not be eligible for Revolving Loan payments.
Your project is subject to the one-year performance certification requirements. By this,
you are required on the date one year after the completion of construction and initial operation of
the subject treatment facilities, to certify, based on your consulting engineer's advisement,
whether or not such treatment works meet the design performance, specifications and the permit
conditions and effluent limitations.
In accordance with the Federal Regulations, the Recipient is required to assure compliance
with the OSHA safety regulations on the subject project. In complying with this regulatory
responsibility, the Recipient should, by letter, invite the Bureau Chief, Education Training and
Technical Assistance Unit, NC OSHA Division, 1101 Mail Service Center, Raleigh, NC 27699-
1101 at (919) 807-2890, to participate in the Preconstruction Conference to assure that proper
emphasis is given on understanding and adhering to the OSHA regulations.
It is the responsibility of the Recipient and the Consulting Engineer to insure that the
project plan documents are in compliance with Amended N. C. G. S. 133-3 (ratified July 13,
1993). The administrative review and approval of these plans and specifications, and any
subsequent addenda or change order, do not imply approval of a restrictive specification for
bidding purposes; nor is it an authorization for noncompetitive procurement actions.
Any addenda to be issued for subject project plans and specifications must be
submitted by the Recipient such that adequate time is allowed for review/approval action
by the State and for subsequent bidder action prior to receipt of bids.
It is mandatory for project facilities to be constructed in accordance with the North
Carolina Sedimentation Pollution Control Act, and, when applicable, the North Carolina Dam
Safety Act. In addition, the specifications must clearly state what the contractors' responsibilities
shall be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable project Permits from the State, including an Authorization to Construct and/or Non -
Discharge Permit.

While rejection of all bids is possible, such action may be taken only with prior State
concurrence, and only for good cause.
A goal of 8% of the contract price is established for Minority Business Enterprise (MBE)
participation in this project, and a goal of 5% of the contract price is established for Women's
Business Enterprise (WBE) participation in this project. The Recipient and Bidders shall make a
good -faith effort to assure that MBE's and WBE's are utilized, when possible, as sources of
goods and services. The good -faith effort must include the following affirmative steps: (a)
including small, minority, and women's businesses on solicitation lists; (b) assuring that small,
minority, and women's businesses are solicited whenever they are potential sources; (c) dividing
total requirements, when economically feasible, into small tasks or quantities to permit
maximum participation by small, minority, and women's businesses; (d) establishing delivery
schedules, and (e) using the services of the Small Business Administration and the Minority
Business Development Agency of the U.S. Department of Commerce. Please note that the
solicitation efforts should include documentable follow-up phone calls.
The Recipient shall comply with the provisions of 40 CFR, Part 7, Subpart C -
Discrimination Prohibited on the Basis of Handicap.
Attached is one (1) copy of the Project Bid Information which is to be completed within 21
days after bids have been received, and submitted to the State for review. Upon review and
approval of this information, the State will authorize the Recipient to make the proposed award.
Do not proceed with construction until the Authority to Award package and the EEO and
MBE documentation/certification have been reviewed, and you are in receipt of our approval, if
a CWSRF loan is desired for project construction.
Two (2) copies of any change order must be promptly submitted by the Recipient to the
State. If additional information is requested by the State, a response is required within two (2)
weeks, or the change order will be returned without further or final action.
One (1) set of the final approved plans and specifications will be forwarded to you. One
(1) set of plans and specifications identical to the approved set must be available at the project
site at all times.
Upon completion of the project construction, the Recipient shall submit a letter confirming
that the project has been constructed in accordance with the plans and specifications approved by
the State. "As -built" plans will need to be submitted with any changes clearly documented on
the plans if the above confirmation cannot be made.

Treatment Process Description
The Louisburg WWTP is an extended aeration system with a permitted treatment
capacity of 1,370,000 gallons per day. It consists of influent screening and grit /grease
removal, an anaerobic mixing zone, two oxidation ditches, two secondary clarifiers, three
tertiary filters, ultraviolet disinfection, cascade aeration, two aerobic digesters and one
effluent reuse station (currently unused).
H. Performance
The Louisburg W WTP facility was compliant throughout the entire twelve month period
with no violations. The facility operated well within design efficiency with some
mechanical breakdowns, but all malfunctions were corrected as needed.
The table below shows plant performance in relationship with NPDES permit limits.

The aquatic toxicity test using 24-hr composite samples of effluent discharged from Louisburg WWTP
has been completed. Louisburg WWTP has an effluent discharge permitted that is 1.37 million gallons
per day (MGD) entering Tar River (7Q10 of 14.0 CFS). Whole effluent samples were collected on April
12 and April 14 by Vicki Webb and Tom Ascenzo for use in a chronic Ceriodaphnia dubia pass -fail
toxicity test. The test using these samples resulted in a pass. Toxicity test information follows.

Dear Mr. Warren
I represent the Construction Grants and Loans Section of the Division of Water Quality,
and attend the Monthly Progress Meetings, conduct Construction Inspections, and review Project
Documentation for your SRG Project No E-SRG-T-09-0078. The progress meeting, site visit,
and construction inspection were held on 3/28/2011.
The assistance of Steve Scruggs P.E. and Wayne Woodlief with AECOM Consultants;
Mike Habig and others with DeVere Construction Company; Ray Patterson and Carlton Tabrow
and other Town of Louisburg staff is greatly appreciated. The project plans, specifications,
permits, construction schedule, funding, testing reports, site visits and inspections by other
agencies, and work done by sub -contractors, SRG and USDA funding requirements, and pay
requests were discussed and are documented by the Engineer's Progress Meeting minutes. I
photograph the construction materials, site, and activities during my site visits and inspections.
The requirements for these vubfically funded vrollects are very important. Information
about these requirements has been provided and they are to be discussed and reviewed at all
progress meetings. Documentation is to be kept on file for possible future audits or review. If

be on right in this photo. Much rock was encountered going
up the line to the left and was blasted.
ACTION ITEMS & NOTES:
• The CG&L Construction Inspection Checklist and SRO
requirements were reviewed.
• Edgewood Road Sewer Pipe and MH installation is more
that 50% completed.
• Plans and Contract being checked for exactly which MH
are included in the SRG part of the project, i.e MH where
Edgewood sections connect to Tar River Interceptor.

Treatment Process Description
The Louisburg W WTP is an extended aeration system with a permitted treatment
capacity of 1,370,000 gallons per day. It consists of influent screening and grit /grease
removal, an anaerobic mixing zone, two oxidation ditches, two secondary clarifiers, three
tertiary filters, ultraviolet disinfection, cascade aeration and two aerobic digesters.
II. Performance
The Louisburg W WTP facility was compliant throughout the entire twelve month period
with no violations. The facility operated well within design efficiency with some
mechanical breakdowns, but all malfunctions were corrected as needed.
The table below shows plant performance in relationship with NPDES permit limits.

I have reviewed the file and spoken with Vick Webb, who recently (11.18.08) inspected the
above referenced facility. Please be advised of the following comments to the permittee,
based on findings during the inspection:
The 1.37 MGD wastewater treatment plant consists of the following components:
mechanical barscreen, aerated grit/grease removal, anaerobic selector, dual oxidation
ditches (BioDeNioPho Process), dual secondary clarifiers, three cell tertiary filter,
ultraviolet disinfection, sludge thickener, aerobic digester, and ultrasonic flow meter.
Update the plant description for the permit to include the cascade aeration.
2. Update the Raleigh Regional Office on the status of the installation of proposed new
equipment, consisting of: mechanical barscreen, septic tank hauler receiving station,
spare digester ejector pump, portable self priming pump, influent and effluent
refrigerated samplers, sludge thickener pumps, clarifier effluent trough cover plates, as
well as replacing the mixers in the oxidation ditch

review processes.
