Thanks for reaching out to us! Please see the attached change of ownership form. I've pre -filled as
much information as I could from our database, so hopefully all you'll have to do is add the missing
information. I see from the County website that the property is co -owned by Mr. Anshooman Aga, so
we will need both of you to sign the Operation and Maintenance Agreement on page 2. Either one of
you can sign the final certification on page 3.

We are the new homeowners of 149 Perfect Moment Dr, Durham NC 27713. We purchased the
house in October 2022 but have just discovered that the septic tank permit has to be transferred
under our name.
Please let me know the documentation or forms that need to be filled out for this to happen.
