I've pre -filled some of the information on the attached form based on the information we already have in our system; please double-check everything for accuracy and fill out any missing information, then return it to me. Since both of you (Mitchell and Nicole) are co -owning this property, we will need both of you to sign the Operation & Maintenance Agreement on page 2. Either one of you can sign the final certification. It also sounded like this house has been built since the last time we issued this permit, so we will likely need to see a copy of the Engineering Certification for the septic installation to verify that everything was installed according to the approved plans. If you're not sure where that Certification is, don't worry; you can send the Change of Ownership form back to me first and then go looking for it. It will take us some time to review everything for the change anyway. Let me know if you have any questions, and have a good rest of your week.
