Thank you very much for contacting us to let us know the property has changed hands. I've attached a Change of
Ownership form with some basic information about your system filled out. Please complete this form. I also see from
County records that both you and Mr. Joel lacovelli are listed on the property deed, so we will need both of you to sign
the Operation and Maintenance Agreement on page 2 of the attached form. Either one of you can sign the final
certification on page 3.
I've also copied the Regional Office to let them know about the change. Let me know if you have any questions and have
