Please reference the Additional Information Request regarding Maple Trace and the following the responses:
Please provide additional information on how the proposed water line and culvert installations will occur in
accordance with Condition 6 of Water Quality Certification No. 3884 and 3890 (Work in the Dry). Please provide
the proposed construction sequence and any applicable details (e.g. cofferdams or other diversion structures) to
ensure compliance with this condition. [15A NCAC 02H .0506(b)(2) & (b)(3)]
Please reference the attached Impact Drawings — Figure 3.5, Coffer Darn Detail.
2. Please provide the proposed permanent and temporary seed mix to be used in impacted areas adjacent to
streams and wetlands. [15A NCAC 02H .0506(b)(2) & (c)(2)]
The seed rnix will be the carne as the rnix approved per the site's erosion and sediment control plan.
3. Please update the site plan to include the most recent jurisdictional determination approved by the Army Corps
of Engineers. [15A NCAC 02H .0502(b)]
We are currently working on obtaining an overall site plan (Attachment A in the application) that reflects current
jurisdictional information. However, the updated wetland data has been added to Impact Map (Figure 3.1).
Additional questions that were presented during the JD site walk are addressed below:
1. Will there be any upgrades to the bridge crossing Reem's Creek?
Yes, NCDOT will be widening the existing bridge as a separate project.
2. What is the slope on the proposed culvert at the stream crossing?
The slope on the culvert is 10.43% which matches the existing grades.
3. Will there be any clearing within the trout buffer (other than where impacts are proposed)?
None is proposed by the owner.
4. Will culverts on site be HDPE or RCP?
The piping will all be HDPE. The headwalls will be precast concrete.
5. Is the proposed stream crossing in the same location as the existing log crossing (the log crossing is labeled as a
culvert on our delineation maps)?
The proposed strearn crossing is located at the existing log/culver location.
Tasha: Attached is an updated Stream and Wetland Delineation Map (Figure 7) that includes an additional wetland
(Wetland 9) that was identified during the site visit.
Zan: I will forward you the updated overall site plan once we receive this information.

The Division of Water Resources (DWR) has performed a review of a 401 Water Quality Certification application received
on August 19, 2015 for the above referenced project. The Division has determined that your application is incomplete and
cannot be processed. The application is on -hold until all of the following information is received:
Please provide additional information on how the proposed water line and culvert installations will occur in

Potentially jurisdictional wetlands and
waters identified on this map have been
located within sub -meter accuracy utilizing a
Trimble mapping grade Global Positioning
System (GPS) and the subsequent
differential correction of that data. GPS
points may demonstrate uncorrectable
errors due to topography, vegetative cover,
and/or multipath signal error.
Note: The illustrated wetland and stream
locations are approximate. These areas
have been flagged in the field; however,
they have not been surveyed. Although
Clearwater Environmental Consultants, Inc.
(CEC) is confident in our assessment, the
US Army Corps of Engineers (Corps) is the
only agency that can make final decisions
regarding jurisdictional wetland and waters
of the US delineations. Therefore, all
preliminary determinations are subject to
change until written verification is obtained.
CEC strongly recommends that written
verification be obtained from the Corps prior
to closing on the property, beginning any
site work, or making any legal reliance on
this determination.
This map was prepared by CEC using the
best information available to CEC at the
time of production. This map is for
informational purposes only and should not
be used to determine precise boundaries,
roadways, property boundary lines, nor
legal descriptions. This map shall not be
construed to be an official survey of any
data depicted.
