RE: Town of Spring Hope
Collection System and WWTP
Dear Ron:
As a follow-up to our discussions and recent meeting at the Raleigh Regional Office
regarding the Town of Spring Hope, the following is an outline of the scope of work
being performed mutually by C.T. Clayton, Inc. and EnviroLink, Inc. The Town of Spring
Hope and Kevin Varnell of Stocks Engineering are providing additional assistance. Our
projected date of completion for the SOC submittal is mid -March of 2008.
1. Collection System:
A. Prioritize problems indicated in previous studies as to contributory rain
flow and cost of repair by completing a field investigation. (EnviroLink)
B. Conduct field inflow isolation and location by manhole segment during at
least two rainfalls of one inch intensity within 12 hours. (EnviroLink)
C. Complete a sewer sampling investigation and locate contributors of
5-day B.O.D. concentrations higher than 250 mg/I on the collection
system map. (EnviroLink)
D. Prepare and/or update Sewer Use Ordinance per NCDENR-DWQ
requirements. (EnviroLink and C.T. Clayton)
E. Complete a table of the problems found to date, recommendation for
repair and give estimated flow to be removed and cost of repair.

F. Smoke Test Collection System (EnviroLink)
G. Prepare a location map for the problems recommended for repair, in
AUTOCAD format. (C.T. Clayton)
2. Pump Station Evaluation:
A. Determine and itemize cost and repairs needed for all pump stations

B. Recommend priority of pump stations to be repaired (EnviroLink and C.T.

3. Wastewater Treatment Plant Evaluation:
A. Evaluate effect the recently completed repairs will have on the efficiency
of the plant (EnviroLink).
B. Itemize cost and schedule for immediate maintenance repairs needed at
the plant (EnviroLink and C.T. Clayton).
C. Recommend an interim solution to increase the efficiency of the plant
and give cost and schedule for the solution (EnviroLink and C.T.
Clayton).
D. Prepare a schematic layout of the plant, showing interim solution
recommendations (C.T. Clayton)
4. Preliminary Engineering Report:
Will be prepared by C.T. Clayton (by first of March, 2008)
5. Special Order of Consent:
Will be prepared by C.T. Clayton (by mid -March, 2008)
Thus far, there has been one rainfall event on February 1" that totaled 1.15 inches;
EnviroLink was able to observe several areas that were suspect to be major inflow
contributors. After conducting a camera inspect of these areas, EnviroLink has found
separated joints, line breaks and numerous areas with root intrusion. A grant was
approved in 2007 for repairs and rehabilitations at the WWTP and in the Collection
System. We are pleased to report the allocation of funds have been re -directed to cover
the areas we've found in the Collection System and at the WWTP that will enable us to
put the Town of Spring Hope in the direction of permit compliance.

RE: Fast Track Sewer Application for Spring Hope Crossing
Dear Daniel:
This letter is in response to the additional information requested for Spring Hope
Crossing. The Town of Spring Hope has contracted Stocks Engineering, PA to
coordinate the team of experts necessary to assess, design, fund, and implement the
sewer improvements needed to allow the existing waste water treatment plant to
operate within the limits of the NCDWQ permit. We have conducted several meetings
with John Holpe, Town Administrator, as well as the current plant operator, Mandy Hall
with Envirolink. The current plant has been in operation for over 60 years, with no
upgrades since it began operation. Typical maintenance has been performed over the
years; however the plant needs a complete rehab to meet the permitted requirements.
The Town of Spring Hope is committed to this project and will assist in any way
necessary to obtain funding for the repairs. With the drought conditions over the past
several months the Town has been able to eliminate Inflow and Infiltration as the
causing factor of the continued violations and will now have to concentrate on the
treatment plant itself.
Stocks Engineering, PA in conjunction with Envirolink has begun the Preliminary
Engineering Study and Report which will be necessary to allow the Town to make an
educated decision on how to proceed and where to obtain funding. The time table
below outlines the items that will need to be done and will allow the Town to properly
plan and budget for the plant repairs.

Timing for bidding and construction may vary depending on the funding available for this
project. The Town of Spring Hope has committed to make any reasonable effort to
perform the necessary repairs to the plant and is willing to use any available resource to

Ladies and Gents,
RRO (Danny, Stephanie, Ron, and I) met with the folks from Spring Hope yesterday,
1/23/2008. The Mayor, Town Manager, area Senator, engineers, operators and developers
were all in attendance. A brief summary of the meeting follows:
They did not bring an SOC application with them as we were led to believe. After much
discussion, it looks as though they should have an application in house by the first week in
March. They have enlisted the aid of C. T. Clayton and Stocks engineering firms.
The Town authorized Envirolink (during the meeting) to go forward with I&I investigations
and repairs.
When questioned if we would consider a reduced flow allocation for the project (Spring
Hope Crossing) that started all this fuss to begin with, we replied no, you can't adequately
treat your current flow, so it stands to reason you can't adequately treat additional flow.

The Division of Water Quality's Raleigh Regional Office has conducted a preliminary review of the
subject sewer extension permit application package. Additional information is required from the Town
before we can proceed with our final review. Please respond to the items in this letter by January 4,
2008:
The Town of Spring Hope wastewater treatment facility has been in violation of its effluent discharge
permit 53 times in the past 21 months. The permit limit violations include 38 violations of BOD 5 day
(20 Deg. C), 13 Fecal Coliform violations and 2 violations of Total Suspended Solids. The permit limit
violations have led to nine (9) separate enforcements covering 19 of the last 21 months in question,
with additional enforcements possible for the months of August and September of this year.
It has been suggested that the continuing BOD and Coliform violations are attributable to Inflow and
Infiltration. A review of your Discharge Monitoring Reports for 2007 show that at no time was a daily
influent flow reported in excess of your plant capacity. Given the Statewide drought conditions, this
would be expected. Your violations of BOD and Coliform, however, continued throughout this
drought period.
The requested flow allocation for the subject project represents 20% of your plant capacity, and an
increase in actual plant influent flow of over 56%, based on your CY 2006 average flow. The
issuance of a sewer extension permit allowing additional flow to your treatment facility would
constitute a violation of North Carolina General Statute 143-215.67, which states in part;
(a) No person subject to the provisions of G.S. 143-215.1, 143-215.108, or 143-215.109
shall willfully cause or allow the discharge of any wastes or air contaminants to a
waste -disposal system or air -cleaning device in excess of the capacity of the disposal
system or cleaning device or any wastes or air contaminants which the disposal system
or cleaning device cannot adequately treat.

The number of NPDES discharge permit violations as stated above indicates a failure of the
treatment facility to adequately treat the waste. The Division of Water Quality will not issue a permit
allowing additional flow to your treatment facility until the Town demonstrates that suitable actions are
being taken on you part or in your behalf to eliminate your NPDES discharge permit violations. You
are also reminded that continued violations could result in a sewer extension moratorium being
placed on the Town per North Carolina General Statute 143-215.67, which states in part;
(c) The Commission may impose a moratorium on the addition of waste to a treatment
works if the Commission determines that the treatment works is not capable of
adequately treating additional waste.
The Raleigh Regional Office staff have suggested that the Town of Spring Hope enter into a Special
Order by Consent in order to address the violations issues with an organized and structured
approach. The Town, however, has refused this regulatory option. Please explain what methods the
Town will utilize to eliminate the treatment plant's continued permit limit violations.
Your Fast Track sewer extension application has been placed on hold pending receipt and review of
your response. Unless we receive your response, in writing, by January 4, 2008, the application will
be considered withdrawn, and the application fee will be forfeited.
Please be advised that entering into a contract for the construction and/or operation of a
wastewater collection system extension without a valid permit is a violation of North Carolina
General Statute 143-215.1 and may subject the Applicant to appropriate enforcement actions
in accordance with North Carolina General Statute 143-215.6. Civil penalties of up to $25,000
per day per violation may be assessed for failure to secure a permit required by North
Carolina General Statute 143-215.1.

Dear Francine
I understand that the Town of Spring Hope has filed an application to the CWMTF for certain
improvements to its sewer system and wastewater treatment plant. The Division of Water
Quality recognizes the need and supports this application.
The Spring Hope system is both hydraulically and biologically overloaded. Over the past 12
months they have reported 29 sewer system overflows discharging an estimated 262,000
gallons of untreated wastewater. In addition, Spring Hope has violated their wastewater permit
effluent limits for BOD 12 times, Fecal Coliform 5 times, and Total Suspended Solids 1 time.
We met with John Holpe, Spring Hope's Town Manager, and their consulting engineers on
March 13, 2007 to ensure that they understood that this performance was unacceptable and
requires fixing. Mr. Holpe assured us that they intend to aggressively solve the problem but it
obviously requires considerable financial resources. The DWQ will continue to enforce permit
and other regulatory requirements.
We fully support the Town's request for funding.

deviated from the plans; however, these deviations will not interfere with the operation of
the dechlorination facility in its intended purpose. The items marked recommended, the
town will do as soon as possible. In several cases they are waiting on ordered materials to
be delivered and on others just for the time to complete those tasks. We have therefore
recommended to the town to start operation of the dechlorination system on August 1,
2006 as required by their permit.

used in the observation of the following construction:
Installation of a dechlorination system utilizing calcium thiosulfate pursuant to the fast track
application received on June 27, 2006, and in conformity with the Minimum Design Criteria for
Dechlorination Facilities.
I certify that the construction of the above referenced project was observed to be built within
substantial compliance and intent of the approved plans and specifications, with the following comments
on deviations from the plans and specifications: These deviations will not affect the operation of the
dechorination system; however, those items marked recommended will be completed as soon as possible.
1. No ceiling insulation. Recommend insulate before winter.
2. Wall insulation existing already, therefore, none installed.
3. No interior door. Exterior door cannot shut. Recommend adding both interior and
exterior doors.
4. Calibration cylinder top/return line can be removed for calibration. Care should be taken
to keep debris from getting in the calibration column. Also, recommend keeping tubes
filled with water to do future calibration and prevent accidentally allowing air into the
lines.
5. Include items in referenced in letter dated July 24, 2006 from Charles w. Chappell, PE.
6. Panel "LP" moved to exterior of dechlorination building.
7. Lighting contactors installed beside panel LP on outside of building
8. Recommend baseboard heater to be added.
9. Pumps and pump receptacles moved from back wall to same wall as baseboard
heater as shown on plans.

Dear Dale:
Per our telephone discussion you are allowed to deviate from the plans
and specifications as follows. These changes were made in the interest
of expediting construction and reducing costs. However safety and code
compliance are in no way jeopardized.
1. Schedule 40 PVC conduit may be used in lieu of galvanized rigid
steel, IMC, EMT, and Schedule 80 PVC.
2. PVC boxes may be used in lieu of cast metal and PVC coated
cast metal boxes.
3. A NEMA 3R circuit breaker load center may be used in lieu of the
NEMA 4X stainless steel panelboard specified. To isolate the
load center from corrosive agents, locate on the building exterior.
The load center may be mounted on the end of the building facing
the Control Building to reduce the length of the feeder circuit.
4. Lighting contactors may be furnished without HOA switches and
indicator lights.
5. A different exterior wall fixture acceptable to the Owner may be
used. We do require that the interior vaportight fluorescent fixture
be furnished as specified although you are not restricted to the
manufacturers listed.

A fast track application for Authorization to Construct dechlorination facilities was
received on June 27, 2006, by the Division. Authorization is hereby granted for the construction
of modifications to the existing Spring Hope WWTP, with discharge of treated wastewater into
the Tar River in the Tar -Pamlico River Basin.
This authorization results in no increase in design or permitted capacity and is awarded
for the construction of the following specific modifications:
Installation of a dechlorination system utilizing calcium thiosulfate pursuant to the
fast track application received on June 27, 2006, and in conformity with the
Minimum Design Criteria for Dechlorination Facilities.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of
NPDES Permit No. NCO020061 issued December 22, 2004, and shall be subject to revocation
unless the wastewater treatment facilities are constructed in accordance with the conditions and
limitations specified in PermitNo. NC0020061.
In the event that the facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Permittee shall take immediate corrective action, including those as may
be required by this Division, such as the construction of additional or replacement wastewater
treatment or disposal facilities.
The Raleigh Regional Office, telephone number (919) 791-4200, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an on site

inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Pursuant to 15A NCAC 2H .0140, upon completion of construction and prior to operation of
these permitted facilities, the completed Engineering Certification form attached to this permit shall be
submitted to the address provided on the form.
Upon classification of the facility by the Certification Commission, the Permittee shall
employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the
wastewater treatment facilities. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by
the Certification Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and
grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each
Class I facility at least weekly and each Class I1, III and W facility at least daily, excluding
weekends and holidays, must properly manage the facility, must document daily operation and
maintenance of the facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall
continue to properly maintain and operate the existing wastewater treatment facilities at all times,
and in such a manner, as necessary to comply with the effluent limits specified in the NPDES
Permit.
You are reminded that it is mandatory for the project to be constructed in accordance
with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North
Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's
responsibilities shall be in complying with these Acts.
Failure to abide by the requirements contained in this Authorization to Construct may
subject the Permittee to an enforcement action by the Division of Water Quality in accordance
with North Carolina General Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by
other government agencies (local, state, and federal) which have jurisdiction.

walk through.
The skimmer arms on both final clarifiers need adjustment. Weirs were not level, causing hydraulics
currents in the clarifiers. There was some algae in the clarifiers weirs and troughs. Pete said they wash
these down 1/week.
Trickling filter distribution arms were plugged in some areas.
Both sludge digesters were near full, and being decanted at the time. Pete said Granville Farms is to come
next week
Effluent was typical of a trickling filter plant; slightly turbid. No samples were taken

A. (2.) Nutrient Limit Waiver
Limits on total phosphorus and total nitrogen have not been included in the current permit. The
Spring Hope WWTP is participating in the nonpoint source trading option outlined in the Tar -Pamlico
Nutrient Sensitive Waters (NSW) Implementation Strategy (Phase II), adopted December 8, 1994.
If compliance with any element of the NSW strategy (or subsequent approved revisions to the
strategy) is not maintained, the Division reserves the right to reopen this permit to include nutrient
limits. If requirements other than those listed in this permit are adopted as part of a future revision
to the NSW strategy, the Division reserves the right to reopen this permit and include those
requirements.

The subject inspection was performed September 8, 2003 by myself. The cooperation of
George Meeks Jr., Grade 2 ORC, during the inspection was appreciated. Findings during the
inspection were as follows:
1. The treatment plant is currently permitted at 0.4 mgd and consists of the following units:
mechanical bar screen, manual bar screen, dual grit chambers, 6" parshall flume with ultrasonic
flow measurement, 6 drying beds (not used, liquid sludge is land applied), 2 primary clarifiers, 2
trickling filters, 2 secondary clarifiers, and 2 aerobic digesters (both near full). The effluent is
pumped approximately 3-4 miles to the Tar River. The outfall line provides contact time for the
chlorine. There was some vegetation growing in the drying beds. Please have this vegetation
removed by hand (no herbicide addition is allowed in these units). All units appeared to be
operating satisfactorily.
2. A new generator is now onsite, and is tested weekly. Mr. Meeks stated the Town has a
portable generator as well.
3. Mr. Meeks stated the Town has purchased a sludge judge for process control testing at the
plant as recommended in prior inspections. You are reminded that clarifier sludge blankets
should be no more than 1/3 of the sidewall depth of the clarifier.
4. The influent flow meter was calibrated November 2002, which is compliant with the
Division's requirement of, at a minimum, annual calibration.

5. The influent sampler is programmed to collect 100 ml of sample at 20 minute intervals, which
is compliant with Division requirements. Mr. Meeks stated the effluent sampler is programmed
to collect composite samples flow proportionally, but did not know the specifics of the program.
Please respond as to how the effluent sampler is programmed (ex. 100 ml of sample collected for
every 1000 gallons, etc.).
6. The effluent appeared free of excess solids. The receiving stream, Tar River, showed no
visible detrimental signs of impact from this discharge.
7. pH meter calibration and documentation appeared satisfactory. As discussed, the chlorine
meter must be calibrated before each day values are reported on the DMR.
The pH meter may need replacing or servicing. Equilibration of the meter takes an excessive
amount of time, which indicates problems with the meter, and validity of the data. Please
investigate this matter as needed.
The Town has not been certified by the Division to perform pH and residual chlorine analyses.
The Town was also notified of this requirement in the July 1, 2003 inspection report. Please
contact the Laboratory Certification Unit at 919.733.3908 for requirements for certification, and
respond to this office on the status of obtaining certification.
8. A review of the ORC logbook showed it to be satisfactory.
9. Mr. Meeks stated sludge is disposed of by Granville Farms. The 503 regulations are
complied with at the WWTP by pH adjustment.
10. Mr. Meeks stated there are 6 pump stations, and only 1 has telemetry. The logbook showed
that all pump stations are visited 7 days/week as required. It is recommended the Town consider
installing telemetry and a generator at each pump station in order to be notified immediately of
problems at each station, to prevent overflows, and subsequent liability for overflows.
11. A review of laboratory and DMR data for May 2003 showed consistent reporting of results.
12. It is recommended that an educational program on eliminating grease into the collection
system be distributed to the residents in the Town.
13. The permit will expire October 31, 2004. You are reminded that a renewal application must
be submitted to the Division at least 6 months prior to expiration.
14. A review of DMR data since the last inspection (April & May 2003) showed no limit or
monitoring violations occurred.

15. It was noted that several residual chlorine values >0.8 mg/1 were reported on the DMR.
While the facility does not currently have a chlorine limit, chlorine can be very toxic to aquatic
organisms. It is recommended that efforts to reduce chlorine concentrations be taken, such as
dechlorination or UV disinfection. As discussed during the inspection, the chlorine dosage may
be set up to be paced according to the flow. This would allow sufficient disinfection while
saving the Town money by not using excess chlorine. Authorization must be obtained from the
Division prior to any modifications made to the system. This office may recommend that a
chlorine limit be imposed in the next permit cycle.
16. A summary of activity to eliminate/reduce inflow and infiltration in the collection system
was recently submitted. No additional information on this matter is required at this time.
17. Analyses are performed by Environment I.
18. Mr. George Meeks Sr. is the backup ORC.
19. It was noted that flow to the trickling filter was almost zero at certain times during the
inspection. As discussed with Mr. Meeks, recirculation rates may need to be adjusted.
20. An effluent flow meter has been installed in order to obtain flow proportional composite
samples. This office did not receive notification of this addition to the facility. Please provide
information on this matter.

Kirk Stafford and I performed a recon at Spring Hope WWTP March 5, 2002. Pete, a worker at
the W WTP, was onsite. George Meeks, ORC, was not onsite at the time of the inspection. A
courtesy OSHA inspection was conducted earlier in the day per Pete.
The 2 trickling filters had growth, but it appeared grayish. Both secondary clarifiers appeared to
have some solids discharging; BOD and TSS grab samples were collected and will be analyzed
by DWQ lab. Both digesters appeared to be 3/4 full.
A new generator has been installed onsite. Pete stated it comes on automatically every Tuesday
at 9:30 a.m.
A replacement line to the plant has been completed.
2 valves on the farthest side from the building at the digesters appeared to have seals leaking.
Pete was instructed to inform George to check into this and stop this leakage.
