Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum
of Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9,
1994 (or as subsequently amended).
Please note that the improvements listed on your August 19 letter have been added to the above -
referenced permit. The temperature monitoring was changed from daily to weekly and a daily
maximum ammonia limit has been added to Section A.(1) of this final permit.
If any parts, measurement frequencies -or sampling requirements contained in this permit are
unacceptable to -you; you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such
demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, Coastal Area Management Act, or any other Federal or
Local governmental permits which may be required.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
The Brunswick County Board of Education is hereby authorized to:

Dear Mrs. Crawford:
In response to the subject letter, the following updated information is provided for the
Waccamaw Elementary School waste treatment facility.
In accordance with NCDENR letter (Alan W. Klimek) dated July 30, 2003; Authorization to
Construct, Brunswick County Schools completed construction of a new facility on August 11,
2004. The following improvements have been installed at the waste treatment, facility and
replace the preexisting components. The septic tanks and grease trap remain as before.
• 10,000 gallon fiberglass dosing tank ,with Orenco Biotube Duplex Pump Vaults and
duplex 1.5 HP, 50 GPM pumps, with automatic controller and alarm system.
• 15 x 15 ft. block building with duplex Ultraviolet disinfection, flow meter, valve
controls, and Y=strainers.
• 3,00 gallon concrete effluent pump tank with dual pumps and controller assembly.
• Distribution box in -line splitter between the re -circulation tank and the effluent pump
tank with dial -a -flow controls.
• New re -circulating sand filter bed consisting of a synthetic liner, raised brick
perimeter walls (36 inches), 24 inches of gravel and state -certified sand, and 2600
linear feet of `Y2 inch PE drip tubing (0.01 gpm emitters @ 24 inches on center)
beneath 6 inches of sand surface.

Subject: Revision of Draft Permit
Donald Dixon of Brunswick County called and requested that the temperature sampling
be changed from daily to weekly. He said that Dean Huntley of the Wilmington Regional
Office agreed with this change. These facilities are Grade 1 facilities.
Also, both facilities have a UV system for chlorine removal therefore there is no need for
this requirement in either permit. They confirmed that they do not have a chlorine
backup.

I would like to modify my comments on the subject draft permit.
I found the 2003 ATC. (We issued it from this office) The
construction was begun this summer and should be finished now
prior to the beginning of school. This will require a change in
the facility description. It will be easiest if you take the
description from the ATC. Please let me know if you don't have
it.

consists of multiple septic tanks on the school grounds, a
4,500 gallon dosing tank, a 6,500 gallon recirculation
tank with dual pumps, PLC control, ultraviolet
disinfection and flow meter, and a recirculating sand
filter bed with four zones.

authorization to construct are to be installed during the
summer of 2004 when the school is not being used.

The school system was under an SOC approximately 10 years ago
to eliminate all discharging systems. A subsurface disposal
system was installed at the school but it failed. It was then
determined that continued discharge to surface waters was the
only reasonable alternative.
PART IV - EVALUATION AND RECOMMENDATIONS
The system has generally performed well since it was upgraded
and given stringent effluent limits about ten years ago. An
Authorization to Construct was issued July 30, 2003, for
significant upgrades to the system including construction of a new
dosing/recirculation tank and rebuilding of the sand filter. This
work is to be done during the summer of 2004 due to some delays.
In the interim, some repairs have been made to the sand filter to
fix damage caused by freezing of pipes. In December, 2003,
effluent limits were violated in the early part of the month prior
to the repairs being made and sand being replaced. Performance
improved after the repair was made and compliance was achieved.
The approved proposed improvements will allow the system to perform
more efficiently while being less prone to damage from freezing.
It is recommended that the permit be reissued for a period of
five years.

North Carolina General Statute 143-215.6(b)(2) provides that Any Person who knowingly makes any false statement representation, or
certification in any application, record, report, plan, or other document files or required to be maintained under Article 21 or regulations of the
Environmental Management Commission implementing that Article, or who falsiffes, tampers with, or knowingly renders inaccurate any
recording or monitoring device or method required to be operated or maintained under Article 21 or regulations of the Environmental
Management Commission implementing that Article, shall be guilty of a misdemeanor punishable by a tine not to exceed $10,000, or by
imprisonment not to exceed sic months, or by both. (18 U.S.C. Section 1001 provides a punishment by a fine of not more than $10.000 or
imprisonment not more than 5 years, or both for a similar offense.)
