This letter is in response to your permit renewal request received on June 27, 2023.
In a letter dated September 11, 2023, Leah Parente of the Non -Discharge Branch asked you to
provide additional information (copy of letter attached) no later than the close of business on October 11,
2023. As of today, the Division has not received all the requested information.
Specifically, the Division has not received a response to the following item:
➢ "Compliance Schedule I.3 of Permit WQ0012709 issued on May 8, 2017, has not been met.
This condition requires the Permittee have the adequacy of the current pumping system
evaluated by a qualified professional and subsequently submit a report of the findings to the
Division's Central and Wilmington Regional Offices. As of the date of this letter, this report
has not been received by the Division. Please provide this report."
Accordingly, the Division is returning your renewal application pursuant to 15A NCAC 02T
.0107(e)(2).
Please be advised that construction and/or operation of the subject facilities without a valid
permit is a violation of G.S. 143-215.1, and may subject you to appropriate enforcement actions
pursuant to G.S. 143-215.6A., 6B., and 6C. The Division may assess civil penalties of up to $25,000
per day per violation for failure to secure a valid permit.
Please submit the requested additional information and associated renewal application within 30
days of receipt of this letter. Failure to provide the requested additional information and the associated
renewal application shall result in a Notice of Violation and may include a civil penalty assessment.

RE: Effluent Flow meter installation
To whom it may concern:
As the ORC of permit # WQ0012709 Wells Pork and Beef Products WWTF I was asked to install and
calibrate an effluent flow meter for the purpose of accurately measuring the wastewater being applied
to the fields.
The Flow meter was installed on 10/4/2023 and after installation a pump test was ran for 10 minutes to
obtain an accurate measurement of the wastewater being applied to the fields. The pump run test was
conducted to verify the new flow meter against flow at a spray head rate of 8.66 gpm. The test area
used had a total of 7 spray heads.
Test results:
10 minute run time= 60.12 gpm on flow meter
60.62 gpm on spray heads
Verified Spray irrigation 60.37 GPM

SUBJECT: WQ0012709 Above Ground Storage Tank Inspection, 709 Croomsbridge Rd, Burgaw NC,
To Whom It May Concern:
Per your request, I performed a visual inspection integrity test on three (3) — 17,000 gallon above ground
storage tanks (AST) on November 29, 2023. There are four tanks total on site, the most northern tank was out of service
and not inspected.
Based upon my visual inspection, I certify the structural integrity of the three 17,000 gal AST. The tanks have no
signs of leakage, infill piping and drain piping are secure and in good condition. Access hatches are properly fastened.
