We are contacting you to determine the status of a wastewater disposal system on your property
that was covered by General Permit NCG550000 [the General Permit for single family residence
domestic wastewater]. We have received notice that the Certificate of Coverage (CoC)for your
property's wastewater disposal system has not been transferred to the current owners. The Division
needs information from you to renew coverage under NCG550000. Please complete the attached
Permit Name/Ownership Change Form and return it, along with a copy of the deed, to the Division

I received a call from Sharon Davis,the former owner of this facility(SFR). She just received the bill for the annual fee
and wanted to let us know that the house was sold to Austin and Sarah Petty(336)848-1626 on July 14, 2016. She
indicated that she had given them the information to get the permit transferred but I assume this has not been done
since BIMs has not been updated. I don't know how long it takes for some of these things to be done after we have
received a name change request. I recommended that she send the invoice back in to the address and indicate that the
property had been sold. That way the billing folks have a record. I put a comment in BIMs to this effect. I guess we
need to follow up either by phone or mail and let them know that they need to get the name change request in . I
assume the billing folks will rebill when BIMs is updated. George, do you know if we need to do anything
differently? Thanks.
