Please find enclosed the Compliance Evaluation Inspection for the site visit conducted on August 11,
2015 by Mr. Ori Tuvia and myself. Your cooperation during the site visit was much appreciated.
An area of concern is that annual effluent testing is not being performed as required by the permit.This
testing is necessary to ascertain whether or not your wastewater treatment system is functioning
properly. As discussed during the inspection,when effluent is not being generated for sampling,you
may need to schedule your effluent collection on days when significant amounts of water will be used at
the residence,such as on wash days. Please note that your prompt attention to these issues will help
avoid future compliance problems.

The wastewater treatment facility serving the subject residence was in good operational condition at
the time of the site inspection. The septic tank has been pumped regularly and chlorine tablets used
for wastewater treatment were present in the disenfection tubes. Effluent was not,observed coming
out of the discharge pipe, and there were no solids deposition at either the outlet or instream. There
was no evidence that gray water was by-passing the treatment system:

The permittee has not sampled the effluent on an annual basis as required by the permit.This was
discussed with the permittee at the time of the site visit. V
