Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of
Agreement between North Carolina and the U.S. Environmental Protection Agency dated May 9,1994 (or
as subsequently amended).
This final permit includes no major changes from the draft permit sent to you on July 21, 2004.
This permit includes a TRC limit that will take effect on June 1, 2006. If you wish to install
dechlorination equipment, the Division has promulgated a simplified approval process for such projects.
Guidance for approval of dechlorination projects is attached.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such
demand is made, this decision shall be final and binding.
'Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This pen -nit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required. If you have any questions concerning this

to receiving waters designated as an unnamed tributary to Mulberry Creek in the Yadkin -Pee

conditions set forth in Parts I, II, III, and IV hereof.
The permit shall become effective December 1, 2004.
This permit and the authorization to discharge shall expire at midnight on September 30, 2008.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of this
permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive
authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and
provisions included herein.
Wilkes County Board of Education is hereby authorized to:
1. Continue to operate an existing wastewater treatment plant consisting of the following
components:

• 9,270 linear feed of force main
• Concrete vault and a blower
The facility is located at the Mountain View Elementary School, 5464 Mountain View
Road, Hays, Wilkes County.
2. Discharge from said treatment works at the location specified on the attached map into an
unnamed tributary to Mulberry Creek which are classified Class C waters in the Yadkin -Pee
Dee River Basin.

This letter is to request review of effluent, up, and down stream sampling.
To the best of my knowledge, the new facility has not been noncompliant
since it was put on line. There has been no adverse effects on this unnamed
tributary that I am aware of.
The forcemain is approximately 3 miles in length and the last '-, mile has a
a very steep grade and is remote and hazardous.
Request that stream sampling be eliminated and further request we be permitted
to sample effluent from wet well.
Your review and cooperation of this request is very appreciated.

Mountain View Elem NC0046418, Wilkes County requested that instream sampling be
eliminated, and further request that the effluent be sampled from the effluent
well. The discharge pipe is 3 miles in length and the last 1/4 mile has a very
steep grade and is remote and hazardous.
