Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly,
we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
This final permit includes no significant changes from the draft permit sent to you on October 24, 2612.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this
letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that
may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended, the

Mayodan WTP
500 East Roach Street
Mayodan
Rockingham County
to receiving waters designated as an unnamed tributary to the Mayo River in the Roanoke River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, H, III and IV hereof.
This permit shall become effective February 1, 2013.
This permit and authorization to discharge shall expire at midnight on May 31, 2017.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
The Town of Mayodan
is hereby authorized to:
1. Discharge decanted conventional water treatment plant filter backwash and filter wastewater from
two alternately utilized settling lagoons, with dechlorination prior to discharge.
The facility is located at the Mayodan Water Treatment Facility, at 500 East Roach Street, in
Mayodan, in Rockingham County.
2. Discharge from said treatment works at the location specified on the attached map into an
Unnamed Tributary to the Mayo River, classified WS-1V CA waters in the Roanoke River Basin.

1. The Division shall consider all effluent TRC values reported below 50 µg/L to be in compliance
with the permit. However, the Permittee shall continue to record and submit all values reported by
a North Carolina certified laboratory (including field certified), even if these values fall below 50
µme•

A. (2.) ACUTE TOXICITY PASS/FAIL MONITORING (QUARTERLY)
The permittee shall conduct acute toxicity tests on a quarterlX basis using protocols defined in the North
Carolina Procedure Document entitled "Pass/Fail Methodology For Determining Acute Toxicity In A Single
Effluent Concentration" (Revised December 2010 or subsequent versions). The monitoring shall be performed
as a Ceriodaphnia dubia 48 hour static test. The effluent concentration at which there may be at no time
significant acute mortality is 90% (defined as treatment two in the procedure document). Effluent samples for
self -monitoring purposes must be obtained during representative effluent discharge below all waste treatment.
The tests will be performed during the months of March, June, September and December. These months
signify the first month of each three month toxicity testing quarter assigned to the facility.
All,toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGA3B.

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later than 30
days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,
the permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating
the facility name, permit number, pipe number, county, and the month/year of the report with the notation of
"No Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences
Section at the address cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately until such time that a single test is passed. Upon submission of a valid test,
this monthly test requirement will revert to quarterly in the months specified above. Assessment of toxicity
compliance is based on the toxicity testing quarter, which is the three month time interval that begins on the
first day of the month in which toxicity testing is required by this permit and continues until the final day of the
third month.
Should any test data from either these monitoring requirements or tests performed by the North Carolina
Division of Water Quality indicate potential impacts to the receiving stream, this permit may be re -opened and
modified to include alternate monitoring requirements or limits.
Should it be determined by the North Carolina Division of Water Quality that the facility's discharge frequency
and duration enable chronic sampling according to either the North Carolina Ceriodaphnia Chronic Whole
Effluent Toxicity Procedure or to Section 8.3, in Methods EPA-821-R-02-013, the permit may be re -opened and
modified to include chronic toxicity tests.
FACT SHEET FOR EXPEDITED PERMIT RENEWALS
This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

The North Carolina Wildlife Resources Commission (Shari Bryant) commented on the
draft permit. Its comments noted the historical presence of endangered, threatened and
significantly rare species in the Mayo River. The NCWRC's concerns about the
discharge were primarily with regard to chlorine in the effluent. As a result, the NCWRC
recommends the facility either move to a non -discharge option for wastewater treatment
or install dechlorination or settling basins to reduce chlorine in the effluent.
In response to the NCWRC's concerns, it should be noted that the facility does use
settling basins for its filter backwash. It also uses dechlorination prior to discharge.
Reported effluent values for TRC have been compliant with the terms of the NPDES
permit. Because of the way the wastewater is managed, flow volume from the facility is
typically very small.
In the past, the WTP staff has considered the connection of the WTP discharge to the
Town's wastewater collection system. The matter was put on hold upon the hiring of a
new town manager, but now that he's become settled in the position, the idea will be
floated again.
This information has been communicated to Ms. Bryant.
It is recommended the permit be issued as drafted.

Hi Shari,
Merry Christmas and Happy New Year to you and yours.
I wanted to follow up with you regarding the NPDES permits for the Madison and Mayodan Water Treatment Plants.
For both of these facilities, the primary concern expressed in your memos regarded the potential toxic effects of
chlorine in the discharge on aquatic life in the receiving streams. Your recommendations were (1) they pursue a non -
discharge option, or (2) use dechlorination or settling basins.
I'd responded earlier regarding Mayodan. This facility does use settling basins for its backwash water and basin
cleaning. It also uses tablet dechlorination for the low volume of flow that is discharged. Reported chlorine residual has
always been compliant since they've been required to monitor for the parameter.
The Madison WTP also uses settling basins for its filter backwash and basin cleaning operations. It does not use
dechlorination at this time. I spoke to the operator there, who told me it wasn't installed because there is no need.
Their basins allow for enough detention time that the chlorine volatilizes before it has to be discharged. This seems very
reasonable; they're only receiving the chlorine from the potable water that's used to backwash. It's not like a WWTP
where chlorine addition is one of the final processes before discharge. Reported chlorine values for this plant are
uniformly low also.
Both WTPs are looking into tying into the Town of Mayodan's wastewater collection system.
Based on the current situations at the WTP's, we are planning to issue the permits as drafted. I hope the discussion
above addresses the WRC's concerns. Perhaps we will soon see both of these discharge eliminated.

Hi Shari,
Thank you for the comments. Hopefully this will ease your mind a bit with regard to this discharge. The facility already
has dechlorination in place. The chlorine residual measured in the discharge is usually very low. The way they manage
the facility and the lagoons on site leads to them discharging only small volumes of wastewater, usually described as "a
trickle," even by our own inspectors.
In the past, because of the low volume of the discharge, the Town has looked into connecting this discharge to the
Town's wastewater collection system. The Town has experienced some recent changes in its management. As a result,
the matter of connection has fallen off the radar as the Town deals with other issues, but it will be reconsidered in the
future.
I'll update the facility description in the permit to note they do dechlorinate.

Biologists with the North Carolina Wildlife Resources Commission have reviewed the subject
document. Our comments are provided in accordance with provisions of the Clean Water Act of 1977 (as
amended), Fish and Wildlife Coordination Act (48 Stat. 401, as amended; 16 U.S.C. 661-667e), North
Carolina General Statutes (G.S. 113-131 et seq.), and North Carolina Administrative Code 15A NCAC
10I.0102.
The Town of Mayodan has applied for a renewal of their NPDES permit to discharge filter
backwash wastewater to an unnamed tributary to the Mayo River in the Roanoke River Basin. According
to the public notice, Total Residual Chlorine is currently water quality limited.
There are records for the federal and state endangered James spinymussel (Pleurobema collina)
and Roanoke logperch (Percina rex); the federal species of concern and state endangered green floater
(Lasmigona subviridis); the state threatened bigeye jumprock (Moxostoma ariommum); the state special
concern notched rainbow (Villosa constricta) and riverweed darter (Etheostoma podostemone); and the
state significantly rare Roanoke hogsucker (Hypentelium roanokense) in Mayo River. Also the
Significant Natural Heritage Area — Mayo River Aquatic Habitat — is located in this section of the Mayo
River.
Chlorine is acutely toxic and can form secondary compounds that are detrimental to aquatic life.
Valenti (2006) concluded the impact of long term exposure to low doses of chlorine may impact juvenile
mussels and reduce the chance of them being recruited to the reproducing population. Due to the
presence of several rare and sensitive species in Mayo River, we offer the following recommendations to
minimize impacts to aquatic resources.
1. Use a non -discharge option for filter backwash wastewater.

Wastewater is produced by wasting the settled solids and filter backwash solids to the
wastewater lagoons. Wastewater is allowed to settle in lagoons with no chemical
treatment, however, dechlor tablets are used to dechlorinate effluent prior to discharging
into receiving stream.

McGill Environmental Systems and removed from site. Solids are then
composted.
