Treatment Facility
Rockingham County
Dear Mr. Carlton:
The final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for
/'\ the construction of a water plant wastewater treatment facility consisting of
dual 325,000 gallon wastewater settling lagoons, inlet and outlet boxes, and
miscellanous piping to serve the Town of Mayodan's water purification plant.
This facility does not require a state licensed operator to be in
responsible charge.
This Authorization to Construct shall be subject to revocation unless the
wastewater treatment facilities are constructed in accordance with the conditions
and limitations specified in Permit No. NC0046302.
Also, enclosed is a copy of WPC Form #50 "Cost of Wastewater Treatment
Works." This form is to be completed and returned to this office within thirty
(30) days after the project is completed.
One (1) set of approved plans are being forwarded to you.
