Dear Mr. Johnson:
Division of Water Quality

Director
March 24, 2011
Dee Freeman
Secretary
Subject: Issuance of NPDES Permit NCO046043
Oak Ridge Military Academy
Class WW-2
Guilford County
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes one major change from the draft permit sent to you on
February 2, 2011:
The nutrient reopener condition has been updated. This reopener may be used by the Division
to further monitor nutrient impacts in the Cape Fear River Basin.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please

Subject: RE: DRAFT pemrit renewal for NCO046043 (Oak Ridge Military Academy WWTP)
The only thing I see that I would add would be in the WWTP description. This facility had a tablet chlorinator installed
during the last permit cycle (early 2008 1 think).
1 notice that Jim Cheshire did not include that in the permit renewal application.
Otherwise, looks good to me.

Addendum: Sludge Management Plan
Oak Ridge Military Academy
NPDES Permit NC 0046043
1) Sludge is generated from Oak Ridge Military Academy WWTP through the
activated sludge process by routinely "wasting" activated sludge into an aerobic
digester.
2) Sludge from aerobic digester is routinely allowed to settle by gravity and
supernatant is decanted from holding tank.
3) Thickened sludge is then removed by septic tank truck and manifest documents
are maintained to verify date of sludge removal, volume pumped, location of
approved facility (i.e. municipal WWTP) receiving sludge, and cost of sludge
hauling/disposal.
FACT SHEET FOR EXPEDITED PERMIT RENEWALS
This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

Most Commonly Used Expedited Language:
• 303(d_) language for, Draft/Final Cover Letters: "Please note that Cane Creek is listed
as an impaired waterbodr on the North Carolina 303(d) Impaired Waters List.
Addressing impaired waters is a high priority with the Division. and instream data will
continue to be evaluated. If there is noncompliance with this permit"s effluent limits and
stream impairment can be attributed to your facility. then mitigative measures mar be
required''. .
• TRC language for Comgliance Level for Cover Letters/Effluent Sheet Footnote:
"°The facility shall report all effluent TRC values reported by a NC certified laboratory
including field certified. However, effluent values below 50 µg/l will be treated as zero
for compliance purposes."
Control Measures Provide exhaust ventilation or other engineering controls to keep:the relevant airborne.
concentrations below their respective occupational exposure' limits. In accordance with good
industrial hygiene and safety work practices, airborne exposures should be controlled to the
lowest extent practicable.
Hygiene measures Wash hands,. forearms and face -thoroughly after handling chemical products, before eating,
smoking and using the lavatory and at the end of the working period.
Personal protection
Eyes Avoid contact with eyes. Safety glasses with side shields.
Skin and body Avoid contact with skin and clothing. Wear suitable protective clothing.
Respiratory Use only with adequate ventilation' Do not breathe vapor or mist. If ventilation is inadequate, use a
NIOSH certified respirator with an organic vapor cartridge and P95 particulate filter.
CAUTION: The protection provided by air -purifying respirators is limited. Use a positive pressure
air -supplied respirator if there is any potential for an uncontrolled release, if exposure levels are
not known, or if concentrations exceed the protection limits of air -purifying respirator.
Hands Wear gloves that cannot be penetrated by chemicals or oil.
The correct choice of protective gloves depends upon the chemicals being handled, the conditions
of work and use, and the condition of the gloves (even the best chemically resistant glove will
break down after repeated chemical exposures). Most gloves provide only a short time of
protection before they must be discarded and replaced. Because specific work environments and
material handling practices vary, safety procedures should be developed for each intended
application. Gloves should therefore.be chosen in consultation with the supplier/manufacturer and
with a full assessment of the working conditions.
Consult your supervisor or S.O.P. for special handling directions
Consult local authorities for acceptable exposure limits.
