Dear Mr. Copeland:
In accordance with your application for discharge permit received on January 23,
1997 by the Division, we are forwarding herewith the subject permit to discharge under the
subject state - NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North
Carolina and the U.S. Environmental Protection Agency dated December 6, 1983 and as
subsequently amended.
Changes to your permit include the following:
Monitoring frequencies for BOD, TSS, NH3, Fecal, and pH have been changed per
updated .0500 regulations.
A toxicity reopener was placed in your permit. This states that toxicity testing may be
required in the future if total residual chlorine effluent levels are not reduced
significantly.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to an adjudicatory hearing upon written
request within thirty (30) days following receipt of this letter. This request must be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General
Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 27447,
Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be
final and binding.
Please take notice that this permit is not transferable except after notice to the
Division of Water Quality. Part. II, EA. addresses the requirements to be followed in case
of change in ownership or control of this discharge. The Division of Water Quality may
require modification or revocation and reissuance of the permit.

be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental
permit that may be required.

Sea Level Extended Care Facility
US Highway 70
in Sea Level
Carteret County
to receiving waters designated as Nelson Bay in the White Oak River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IV hereof.
This permit shall become effective October 1, 1997.
This permit and authorization to discharge shall expire at midnight on July 31, 2002.

SUPPLEMENT TO PERMIT COVER SHEET
Sea Level Extended Care Facility
is hereby authorized to:
1. Continue to operate an existing wastewater treatment plant (Outfall 001) consisting of three
septic tanks each followed by an influent pump station, a flow splitting unit, a 0.014 MGD
extended aeration package plant (dual 0.007 MGD units), dual tertiary filters, chlorination
facilities, and a flow measuring device located at Sea Level Extended Care Facility, US
Highway 70, in Sea Level, Carteret County (See Part III of this Permit), and
2. Discharge from said treatment works at the location specified on the attached map into Nelson
Bay which is classified Class SC waters in the White Oak River Basin.

The pH shall not be less than 6.8 standard units nor greater than 8.5 standard units and shall be monitored weekly at the effluent by grab

needed at the facility. Pat suggested that they submit plans and specs and receive an ATC.

Chlorine levels are high - max last 2 years = 3236 and 2561 µg/l, avg last year = 2404 µg/1.
One Fecal fail 10/96, two BOD fails 4 & 5/95.
The facility stopped monitoring outfall 002 in Sept. '90. Has this outfall ceased discharge?
Facility discharges to Nelson Bay, are there any U & D monitoring locations for turbidity?
Recommend renewal of NPDES permit with the above changes.

4/15 Went on a site visit with Pat Durrett. ORC was Don Copeland, Jr. Changes he will make:
Take the effluent sample farther down the effluent pipe.
Try to reduce the amount of chlorine added and not vilolate fecal limit.
002 is not a discharge, the flows were combined. I will take out the 002 eff page.
4/29 Received SL report from RO.
Concur with putting a toxicity reopener in the permit.
Also request that facility submits an ATC for additional sludge holding.
