Inspection Summary:
Inspection was conducted on September 5, 2023. Permittee was not available on site. The following are the observations:
1. POM (pumping operation and monitoring) plan document for this for mine dewatering was not received along with this
permit application. POM guidelines can be found in section G-2 of NCG02 permit draft as given below:

2. Water accumulation with high turbidity was observed on this site along with steep slopes leading to erosion as shown in
the picture attached. This turbid water is likely to bust benchmarks if released into the surface waters. This issue needs to
be addressed in the POM describing what control measures will remove pollutants and turbidity from dewatering runoff from
this site. Permittee to address turbidity issues before dewatering from this mne site.
3. In this permit application,the plot plan indicated 'sheet flow across wetland'through a 'level spreader', but the design of
this level spreader was missing. Instead, we saw a ditch at the site where location of this level spreader is indicated on the
plot plan. This ditch may lead to point discharge into surface waters. Design (dimensions, location, drawing etc.)of such a
level spreader needs to be submitted for approval.
Approval of the POM Plan may be required prior to coverage according to this General Permit NCG02. Therefore, issuance
will be considered after above tasks are completed.
