Director Preston Curtis 8200 East Nash Street Spring Hope, NC 27882 Mr. Curtis, NORTH CAROLINA Environmental Quality December 8, 2023 Subject: Compliance Evaluation Inspection Permit # WQ0001122 Facility: Curtis Insulation CLRS Nash County On December 4, 2023, Jim Westcott from the Raleigh Regional Office visited your closed loop recycle system to evaluate compliance with the subject permit. The assistance of Mr. Curtis at the time of the inspection was appreciated. The facility is permitted for the operation of the operation of a 900 GPM wastewater treatment and closed loop recycle facility consisting of a 2.15 MG settling lagoon with recycle of wastewater back into the manufacturing facility; and all associated piping, valves, controls, and appurtenances with no discharge of waste to surface waters. The permit, which was issued March 25, 2019, is effective through January 31, 2025. Division records indicate the required annual fee for the period ending December 31, 2023, due January 01, 2023, has been paid. Findings during the inspection were as follows: 1. Treatment system Operation: The wastewater treatment system shall be effectively maintained and always operated so there is no discharge to surface waters, nor any contravention of groundwater or surface water standards. At the time of inspection, the permittee gave a comprehensive tour of the system components. The treatment system lagoon is currently not in service and has been offline since 2002. The discharge lines from the lagoon have been removed. The lagoon freeboard was at the required 2' 2. Proper Operation and Maintenance: The system shall be inspected monthly to make certain of the proper operation of the drip irrigation system and that the soil is assimilating the disposed treated wastewater with no surface runoff, and that no objectionable odors are being generated. Any visible signs of a malfunctioning system shall be documented, and steps taken to correct the problem. At the time of inspection, the perimeter fencing was secured. The permittee is required to notify the division in writing prior to future operations to ensure a startup inspection is conducted. Sampling of monitoring wells MW-1, MW-2 and MW-3 are required to be sampled within three months prior to resuming the closed -loop recycle operations and each November annually thereafter should the facility be in service. Please continue to periodically inspect and evaluate the wastewater drip irrigation system components are properly maintained and operating as designed. Maintain all associated operational records onsite for a period of five years as required by the subject permit.
