Description of existing facilities: 1,000 gallon baffled septic tank, a 360 square foot (ft2) subsurface sand
filter, a tablet chlorinator, a 2,400 gallon storage/pump tank with an irrigation pump, and audible/visual
high-water alarms, a 0.23 acre spray irrigation area with three full-circle 65-foot diameter spray heads,
and all associated piping, valves, controls, and appurtenances.

IV. ADDITIONAL REGIONAL STAFF REVIEW ITEMS
While regional office staff does not recommend denial of the permit renewal, it is recommended that
communication be established with the permittee prior to issuance. RRO staff has been unable to contact the
permittee at the phone number provided and has received no response to letters sent in the mail. The system is not
compliant and is in need of repair as detailed in the attached inspection letter/NOV.
